| Objective Membranous nephropathy is the most common cause of primary nephrotic syndrome in middle-aged and elderly people.Idiopathic membranous nephropathy(IMN)is the main form of membranous nephropathy.With the discovery of the mechanism of podocyte action,podocyte injury is related to autophagy level.Increasing autophagy level can reduce the damage of podocyte and target autophagy has the possibility of treating IMN.Tripterygium wilfordii polyglycosides have direct podocyte protection effect in treating podocyte disease.Exploring whether Tripterygium wilfordii can alleviate podocyte injury by upregulating autophagy is of great significance.Methods In the first part of clinical study was to observe the changes of autophagy in different pathological stages of IMN.The expression of autophagy related protein in IMN podocytes and the relationship between autophagy and IMN podocyte injury proteinuria and progression of renal disease were studied.The secound part of the experiment,the model of glomerular podocyte injury induced by PAN was used.Flow cytometry and transmission electron microscopy were used to observe the model of podocyte apoptosis induced by PAN.The effects of tripterygium wilfordii polyglycosides on the repair of podocyte injury and the signaling pathway of tripterygium wilfordii polyglycoside regulating autophagy in podocyte.Results The model of podocyte injury was successfully established by PAN treatment.Pan could down-regulate autophagy level and induce podocyte apoptosis.The change of autophagy level is closely related to podocyte injury.Tripterygium wilfordii polyglycosides can inhibit cell damage by promoting autophagy.Tripterygium wilfordii polyglycosides can promote autophagy by inhibiting mTOR signaling pathway.The level of autophagy activity in IMN patients with basic level of autophagy was down-regulated and correlated with clinicopathological stages.There is no clear correlation between glomerular filtration rate and renal function.Conclusion Tripterygium wilfordii polyglycosides play a protective role in the pathogenesis of autophagy by inhibiting mTOR signaling pathway.The level of autophagy activity in IMN patients is down-regulated and correlated with clinical pathological stages. |
