The Status Of Physical Activity Among People With Cancer Receiving Chemotherapy And The Effect Of Exercise Intervention On Sleep,Fatigue And Quality Of Life

Objective:Cancer has become a major disease endangering the health of Chinese residents.Health issues of cancer patients,such as sleep,fatigue and quality of life,are also getting more and more attention.This study investigates the current status of physical activity,sleep,fatigue,and quality of life of cancer patients receiving chemotherapy,and explores the relationship between physical activity and sleep,fatigue,and quality of life.We also explore the effects of exercise intervention on sleep,fatigue and quality of life of cancer patients receiving chemotherapy,try to provide some suggestions and guidance for improving the health of cancer patients.Methods:(1)People with cancer receiving chemotherapy at Peking University Third Hospital between July 2019 to January 2020 were surveyed.We collected data including sociodemographic characteristics and information about physical activity,cancer-related fatigue,and quality of life by using International Physical Activity Questionnaires(IPAQ),Pittsburgh Sleep Quality Index(PSQI),Cancer Fatigue Scale(CFS),and The European Organization for Research and Treatment of Cancer quality of life questionnaire(EORTC QLQ-C30).We examined associations of Physical Activity(PA)with sleep,cancer-related fatigue and quality of life by using multivariable linear regression models,adjusted for several sociodemographic variables(age,sex and so on)and BMI.(2)A total of 32 cancer patients who meet the criteria for inclusion and exclusion were selected.According to the exercise guidelines,these patients were subjected to 12 weeks of combined aerobic and resistance exercise intervention to explore the effects of exercise intervention on cancer patients’ sleep,fatigue and quality of life.Results:(1)The weekly PA median value of cancer patients receiving chemotherapy was 808.5(462.0,1848.0)MET-minutes/week,which is much lower than the normal weekly PA median value of 2852 MET-minutes/week.74.7%patients meet the recommendation of 150 min/week of Physical Activity(PA).There are 6(8.0%)of patients are classified as ’high’ PA level,45(60.0%)are classified as’moderate’ PA level and 24(32.0%)are classified as ’low’PA level.(2)Among the 75 cancer patients receiving chemotherapy in this survey,54.7%of the survey subjects had a PSQI score of>5,and their sleep status was poor.The main problems among them were falling asleep time,sleep efficiency,sleep time,and sleep quality.58.7%of the surveyed subjects were fatigued,of which emotional fatigue was the most serious,followed by physical fatigue and cognitive fatigue.The Global Health Status of the survey subjects is good,and the overall performance in the functional areas and symptom areas is good.However,the two individual values of the social function in the functional field and the economic status in the symptom field are not as good as the reference value.(3)When PA was defined as a continunous variable in the multiple regression model,cancer patients with higher physical activity values have lighter physical fatigue,β=-0.097,that is,for every 100 MET-minute/week increase in PA value,physical fatigue score will drop 0.097 points;cancer patients with higher physical activity values have lighter symptoms and better global health status,β is-0.252(P=0.036)and 0.437(P=0.013),that is,for every 100 MET-minute/week increase in PA value,the symptom score will drop 0.252 points,and the global health status score will increase 0.437 points.When PA is defined as a dichotomous variable in the model,the physical fatigue score of the substandard physical activity group is 3.07 points higher than that of the standard group(P=0.022),and the total fatigue score is 4.88 points higher than the standard group(P=0.027).The average symptom is 11.24 points higher than that of the standard group(P=0.001),and the global health status is 12.61 points lower than that of the standard group(P=0.011).(4)After 12 weeks of exercise intervention for cancer patients,the physical fatigue score of the subjects decreased from 8.30 ± 4.39 before exercise to 6.74±4.49 after exercise(P=0.036);the overall fatigue score was changed from before exercise of 19.37±7.36,which dropped to 16.19±8.18 points after exercise(P=0.012);The subjects’ average functional score increased from 81.26± 12.11 to 86.57± 12.22(P=0.045);the average symptom score decreased from 19.27±9.44 to 13.19±9.04(P=0.006).Conclusions:(1)Patients with cancer receiving chemotherapy have poor sleep quality,severe fatigue,better global health status in quality of life,and better overall performance in functional areas and symptom areas.(2)People with cancer receiving chemotherapy are physically inactive,the level of PA is lower than the normal.(3)The physical activity value is higher,and the cancer patients who meet the physical activity standard have less physical fatigue and overall fatigue,less symptoms and better overall health.(4)Exercise can improve the fatigue and quality of life of cancer patients receiving chemotherapy.
