| ObjectiveBased on rank of death causes,disease of burden and trends of change in residents of Binyang county over a six-year period between 2013 to 2018,we to identify the major diseases that pose serious health risks for residents and to identify reference basis assisting prevention and control strategies.MethodsData collected in the death surveillance of residents in Binyang county,Guangxi Zhuang Autonomous Region between 2013 to 2018.Death information in the population were encoded according to the International Classification(ICD-10).Data was collated and analysed by Excel 2016 and SPSS 24.0software.The frequency and composition of causes of death were described by rate and proportion respectively.APC were calculated and hypothesis testing was done with linear regression to understand trends.Mortality and probability of premature death for chronic diseases were emphasized.life expectancy and cause eliminated life table were fully utilized.PYLL and other indicators from all causes of death for residents of Binying county over a six-year period were calculated to identify priorities for health prevention and control.There is also a focus on the disease burden of diabetes,with statistical indicators including the calculation of DALY.Results1.The total number of people monitored for causes of death in Binyang county from 2013-2018 was 4875017,with a slightly larger proportion of males(51.38%)than females(48.62%).2.The count of deaths was 38652,with crude and standardised mortality rates of 792.86/10~5and 707.59/10~5respectively.Crude mortality rate was931.66/10~5in male.The standardised mortality rate of men was 857.93/10~5.Among women,the annual crude mortality rate was 646.17/10~5.The standardised mortality rate was 551.47/10~5in female,with a statistically significant difference(p<0.01).Age-specific death rate increased with age in a"J"pattern.3.The order of the top 5 causes of death was cerebrovascular disease,malignant neoplasm,heart disease,respiratory disease and injury.The order of the top 5 causes of death for male residents were malignant neoplasm,cerebrovascular disease,heart disease,respiratory disease,and injury.For female residents,the order of the top 5 causes of death were cerebrovascular disease,heart disease,malignant neoplasm,respiratory disease,and injury.4.The mortality rates of 40.37/10~5,687.46/10~5and 62.50/10~5were estimated in the first third major disease category,chronic non-communicable diseases and injury respectively.Furthermore,dwellers accounted for 5.09%,86.71%and 2.52%of illness of death in the first third major disease category,NCD and injury respectively.NCD of both males and females had the largest proportion,accounting for more than 85%and increasing year by year.5.The changes in mortality rates of malignant neoplasm and cardiovascular diseases were stable among residents of Binyang county between 2013 to 2018.A downward trends in the crude mortality rate because of respiratory diseases was observed,which decreased from 77.09/10~5of 2013 to 56.08/10~5of2018.while the crude mortality rate due to diabetes had an increasing trend,with an APC in excess of 15%.6.The life expectancy of population ranged from 73.61 to 76.33 years old;69.86-72.60 years old for males and 77.57-80.95 years old for females.7.Life expectancy after removing malignant neoplasm,chronic respiratory disease,diabetes included cardiovascular disease for the whole population during 2013 to 2018 fluctuated 76.80-79.79,74.72-77.29,73.76-76.53 and82.55-87.82 years old respectively.Life expectancy grew by 3.19-3.68 years old,0.76-1.11 years old,0.09-0.26 years old,7.85-11.49 years old.The rate of life expectancy added 4.33%-4.83%,1.00%-1.51%,0.12%-0.34%,10.49%-15.05%in that order.8.The probability of premature death from chronic respiratory diseases and cardiovascular and cerebrovascular diseases from 2013 to 2018 in the whole population of binyang county went down year by year.the probability of premature death about malignant neoplasm and diabetes mellitus did not change much during these years.While the major chronic diseases were reduced from21.63%in 2013 to 20.47%in 2018.9.The total PYLL in Binyang county from 2013 to 2018 was 392,619person-years,with an AYLL of approximately 19.03 years/person and a PYLLR of 83.97‰;the standardized PYLL was approximately 393,394 person-years and the standardized AYLL was 19.07 years/person,respectively;the standardized PYLLR was 84.14‰.The top five PYLL were malignancy,injury,cerebrovascular disease,heart disease as well as respiratory disease;the top three AYLL were perinatal disease,congenital anomalies and obstetric disease.10.PYLLs for diabetes mellitus were 4,530 person years,with males higher than females at 2,970 and 1,560 persons years,respectively,among inhabitants of binyang county in 2013-2018.the AYLL was 11.92 years/person among population and the PYLLR was 0.97 per 1,000.11.DALYs for diabetes was 22,698 among the whole population of binyang county in 2013-2018,of which 16,077 were in males and 6,622 in females,with males being higher than females.The DALYs for diabetes were mainly concentrated in the age group greater than 30 years and beyond.The rate of DALYs for diabetes among the whole population of residents in Binyang county increased from 3.18 person-years/1000 in 2013 to 4.10person-years/1000 in 2018.Conclusion1.From 2013 to 2018,the mortality rate of male residents in Binyang County was higher than that of female residents,and the life expectancy was lower than that of female residents.2.The order of the top 5 causes of death for residents of binyang county from 2013 to 2018 were cerebrovascular disease,malignant neoplasm,heart disease,respiratory disease and injury.3.The mortality rate of malignant tumors and probability of premature death changed steadily,the mortality rate of cardiovascular and cerebrovascular diseases changed little,and the probability of premature death tended to decline,while the mortality rate of NCD and the probability of early death tended to decline;The mortality rate of diabetes has an upward trend,and the probability of premature death has no big fluctuation.Cardiovascular disease is the best effect to eliminate the cause of death and life expectancy.4.The disease burden caused by NCD and injuries is heavier than other diseases.In addition,the burden of perinatal diseases,congenital abnormalities and obstetric diseases was also heavy.The disease burden caused by diabetes in men is higher than that in women,and the heavier burden is in people over 30years old. |
