ObjectivesTo understand the current situation of the causes of death and to analysis the composition of the main death causes of and the changes of the trend of death spectrum for residents in Feicheng City by analyzing the monitoring data of death from 2012 to2017 was analyzed.The burden of major diseases was analyzed to identify the main health problems of the residents in Feicheng City.So as to provide scientific data for rational allocation of medical and health resources,establishment the strategy of disease prevention and control,and enhancement of health status of the residents in Feicheng City.MethodsWe collect the death data of Feicheng City between 2012 and 2017 from Shandong Province Chronic Disease Surveillance System(Death Cause Monitoring System).The basic information of residents was obtained from Public Security Bureau of Feicheng City.The underlying causes of death were coded using the International Classification of Diseases(ICD)-10.The main health problems of the residents in Feicheng City were analyzed by mortality rates of specific diseases,the proportions of the causes of death and the sequence of death cause among the residents.The epidemiological parameters,such as life expectancy,potential years of life lost(PYLL)and disability-adjusted life years(DALY)were used to measure the main disease burden of residents in Feicheng City.The Pearson Chi-square tests were used to compare the mortality rates and proportions between groups.The statistical significance level was 0.05.Results1.Basic information of population in Feicheng City: The total population in Feicheng City was 984,432 in 2012 and 991,680 in 2017,with an increased number of 7248.The number of persons aged 60 and above in Feicheng City increased by 61,482 from 2012 to2017.The total burden coefficient of population in Feicheng City increased by 7.07% in the five years.2.Death data of residents in Feicheng City: From 2012 to 2017,the mortality rates among men were 7.91‰、8.17‰、7.97‰、8.09‰、8.54‰、8.39‰,which were higher than those of the mortality rates among women(6.07‰、6.10‰、6.22‰、6.08‰、6.60‰、6.67‰).From 2012 to 2017,the major institutes of disease diagnosis in Feicheng City were secondary hospitals(51.66%-63.99%),and tertiary hospitals(23.48%-37.96%).The most of methods of disease diagnosis were clinical and physicochemical examination,accounting for more than 60% of disease diagnosis methods.The results of mortality analysis of the three major causes of death of residents in Feicheng City showed that the mortality of chronic diseases was the highest in 2012-2017(630.52-695.75/100,000),and the mortality rates of infectious diseases,maternal and infant diseases and nutritional deficiency diseases increased by 4.36/100,000 from 2012 to 2017.While,the mortality of residents due to unknown causes decreased year by year.The mortality rates of chronic diseases among men(703.61-768.01/100000)were higher than those of women(556.41/100000-622.50/100000),chronic disease mortality in women(556.41/100000-622.50/100000).3.Analysis on sequence of death cause in Feicheng City from 2012 to 2017: The leading causes of death of Feicheng City were,from the highest to the lowest,malignant tumors,heart disease,cerebrovascular disease,and respiratory diseases and injuries,and the causes of death did not change within five years.The cause of death in men was consistent with the total population,while heart disease was the leading cause of death in women.4.Results of life expectancy of residents in Feicheng City from 2012 to 2017: The life expectancies of the residents were 77.06,76.75,78.23,78.96,78.57 and 78.91 years old,among which the life expectancies of men were 74.31,73.79,74.98,75.64,74.43 and76.13 years old,the life expectancies of women were 80.02,79.93,81.54,82.43,82.35 and81.76 years old,respectively.5.Analysis on the PYLL due to major diseases of the residents in Feicheng City: from2012 to 2017: The PYLL due to major diseases of the total population in Feicheng City were cancers(84939.3-91759.7 years),cardiovascular and cerebral vascular diseases(44978.7-49528.3years)and respiratory diseases(4253.8-5642.5 years).6.Results of DALY: The mean DALYs were 85671.29 person years、87210.31 person years、85446.95 person years、84669.73 person years、89385.86 person years、87342.29 person years、87342.29 person years from 2012 to 2017,50991.50 person years、52360.79 person years、50659.83 person years、50924.50 person years、53688.75 person years、50871.64 person years person years for men,34679.78 person years、34849.52 person years、34787.13 person years、33745.23 person years、35697.11 person years、36470.65 person years for women in these five years.Conclusions1.The aging trends in Feicheng City is increasing,which leading to heavy burdens.More attention should be paid resolve the prominent aging problem.2.Chronic diseases have been causing major loss of health for the residents in Feicheng City.The mortality rates of chronic diseases are higher among elderly men compared to women.Therefore,the prevention and treatment of chronic diseases are needed to be strengthened.3.In recent years,the proportion of the patients who attended in secondary hospital and tertiary hospital increased,and rate non-hospitalization decreased,suggesting consciousness of disease treatment was strengthened,and the health investment increased.The difficult to see a doctor situation has been resolved.The relevant departments should further clarify the hospital division of patients,and allocate health resources more rationally,resulting in promotion of the reasonable medical treatment.4.Cancers and heart diseases were the major causes of death in Feicheng City,and the top cancers in Feicheng City are esophageal cancer,lung cancer,gastric cancer and liver cancer.The main diseases are needed to be prevented and treated in risk populations,and early screening of cancers should be undertaken to realize early detection and treatment of these diseases.5.The institutes for disease control and prevention should strengthen the analysis and utilization of cause-of-death surveillance data,strive to achieve data sharing,provide data services for local government,especially provide data for health care,scientific research and public service,and truly reach the scientific usage of cause-of-death surveillance data. |
