Study On Yu Yingao’s Medical Records Of Treating Menopathy And Leucorrhea Disease

1 Research purposesIt is planned to collect,sort out and study Yu Yingao’s clinical medical records of treating retarded-menstruation,hypomenorrhea,amenorrhea,metrorrhagia,perimenopausal disorders,leukorrheal diseases and taking down these six diseases.Analyze his medication law,clinical syndrome addition and subtraction and other experience in treating these six diseases by using literature research method,case analysis method and data mining method.To provide the academic experience of famous traditional Chinese medicine for the diagnosis and treatment of gynecological menstrual diseases and vaginal diseases.2 Research contentsThis paper mainly carries out research from four aspects.The first part summarizes the ancient doctors’ understanding of the etiology and pathogenesis of various diseases,and emphasizes that Yu Yingao paid attention to combing the origin of diseases,reflecting his academic thought of tracing causes and treatment.The second part is based on the real-world electronic case of Yu Yingao outpatient department,and uses the method of data mining to obtain the drug frequency of each disease,the nature,taste and meridian of the drugs used,the drug combination,association rules,core combination,the use range of core drugs and their doses and other information.The third part combines the data mining results with real medical cases to analyze Yu Yingao’ s experience in clinical treatment,prescription and addition and subtraction of drugs.The fourth part selects clinical cases to illustrate Yu Yingao’ s diagnosis and treatment ideas through a detailed interpretation of the real and complete treatment process.3 Research methodsLiterature research method:Use the databases of CNKI,VIP and Wanfang to collect the literature related to the graduation thesis,the search keywords are "Yu Yingao","Yu Yingao+Gynecology",and "retarded-menstruation","hypomenorrhea","amenorrhea","metrorrhagia","perimenopausal disorders","leukorrheal diseases";Use the library of Chinese Academy of traditional Chinese medicine,National Library and other book resources to consult relevant theoretical resources.Collect and sort out Yu Yingao’ s notes,reading experiences,treatises,papers and research results of his academic successors in the diagnosis and treatment of gynecological diseases.Case analysis method:Sort out the medical records with good curative effect,characteristics and reference value,and analyze them from the perspectives of pathological pathogenesis,therapeutic principle therapeutic method,medication characteristics,core prescriptions and prognosis.Data analysis:From the clinical cases of Yu Yingao’ s diagnosis and treatment of menstrual diseases and leukorrheal diseases diseases from January 2012 to July 2019,the medical records that meet the inclusion criteria are selected,and the data mining and analysis are carried out by using the.TCM Inheritance Support System V2.5 developed by the Institute of tradi tional Chinese medicine of the Chinese Academy of Sciences of traditional Chinese medicine,so as to summarize and refine Yu Yingao’s ideas,medication laws and methods of diagnosis and treatment of gynecological diseases Experience characteristics.This paper uses thr ee methods:literature research method,data analysis method and case analysis method to mine and analyze the data of diseases with 20 cases or more,such as retarded menstruation,hypomenorrhea,amenorrhea,perimenopausal disorders,leukorrheal diseases;For diseases wi th less than 20 cases(metrorrhagia),literature research and case analysis were used.4 Research resultsYu Ying’ao believes that the common causes of Retarded-menstruation are liver and kidney yin deficiency and liver qi depression.Treatment to nourish the yin of the liver and kidneys,thin liver and blood nourishment.The core drugs are chinese thorowax rootl0g,chinese angelical0一 15g,nutgrass galingale rhizomel0～12g,prepared rehmannia root10～30g,radix salviae miltiorrhizae rootl5～20g,unprocessed rehmannia root10～30g,debark peony rootl0～18g.The four qi and five flavors of commonly used drugs are mainly warm and bitter.The highest frequency of menstruation is the liver,followed by the spleen.Commonly used pairs of medicines are chinese thorowax root with nutgrass gal ingale rhizome,prepared rehmannia root with chinese angelica and so on.Menorrhagia is often caused by insufficient kidney essence and kidney qi,liver depression and diarrhea disorders,and it is advisable to nourish the yin and supplement the kidneys,and to loosen the liver and blood.The core drugs are unprocessed rehmannia root 15～20g,prepared rehmannia root 10～30g,chinese angelica 10～12g,chinese thorowax root 8～10g.The four qi and five flavors of 15g,unprocessed rehmannia root 15～50g,figwort root 15g,chinese thorowax root 10g,nutgrassgalingale rhizome 8～10g,chinesegentian 6～10g.The four qi and five tastes of commonly used drugs are mainly cold,warm and bitter.The highest frequency of menstruation is the liver,followed by the spleen.Commonly used combinations are prepared rehmannia root and unprepared rehmannia root,unprepared rehmannia root with figwort root and so on.Yu Ying’ ao believes that the next disease is caused by spleen deficiency and dampness,and i t is al so accompani ed by menst rual disorders.Therefore,through the same treatment,to relieve the liver and qi,to supplement the kidneys and nourish the blood.The core drugs are chinese thorowax root 10～12g,processed rehmannia root 10～30g,unprocessed rehmannia root 10～30g,radix astragali preparata 15～36g,indian bread 15～20g,dried tangerine peel 4～6g.The four qi and five flavors of commonly used drugs are mainly warm and bitter.The highest frequency of conclusion1.The drugs used are light and effective,and take care of the spleen and stomach.For the patients with miscellaneous diseases whose condition is relatively stable and the syndrome changes little,Yu Yingao often prescribes 30-50 doses of drugs to ease the conditioning.In addition,he prescribes no more than 15 medicines,and the dose of each medicine is between 10-20g and no more than 50g at most.Most of the drugs he chooses are mild in nature and taste.He often adds drugs that protect the Qi of the spleen and stomach,which has the characteristics of light and flexible use of drugs.2.At the same time,it regulates qi and blood,and uses cold and warm medicines.Gynecological patients with a large number of miscellaneous diseases often show the clinical characteristics of qi and blood disorders and mixed cold and heat.In view of these two characteristics,Yu Ying’ ao uses simultaneous conditioning of qi and blood,and uses cold and warm medicines to adjust the yin and yang of qi and blood.The objective data show that the three flavors of chai hu,angelica and ripe dihuang are all used in the top ten drugs of Yu Yingao’ s treatment of menstrual disease and the frequency of medication under the belt disease,and the four qi used in the treatment of late menstruation,menorrhea,amenorrhea and the drugs used under the belt are mainly warm,cold and flat,and the evidence before and after menopause is mainly cold,warm and flat,and the method of cold and temperature is used together.3.Treat menstrual diseases from the liver and spleen.Yu Yingao believes that irregular menstruation is closely related to the liver and spleen,so he pays attention to the treatment from the liver and spleen,and the specific treatment rules focus on the pathogenesis.If the liver is mainly depressed and qi knot,the treatment is to soothe the liver and promote qi;Those with deficiency of liver yin and blood will soften the liver and nourish yin,activate blood and nourish blood;If regulating menstruation on the basis of treating leucorrhea.Use coix seed,Atractylodes macrocephala,Poria cocos and other traditional Chinese medicine that benefit water and dampness,together with Eupatorium adenophorum,Salvia miltiorrhiza,suberect spatholobus stem,etc.5.Make good use of classic recipes.Taking into account the weak constitution of women,Yu Yingao often uses small prescription with a simple flavor and a moderate dose to minimize damage to the vital qi.Because the pathogenesis of menstrual disease and vaginal discharge is often Qi and blood disorders,so he often uses a formula that not only enters the qifen and xuefen,but also harmonizes the qi and blood.
