| In the context of the medical market,which is characterized by the problems of "difficult and expensive access to medical care",maternal and child health care institutions are increasingly affected by the internal and external environment,and their profit-seeking behavior is becoming more and more serious.Maternal and child health care institutions are relatively special subjects in the medical and health service system.Not only do maternal and child health care institutions assume important public health functions,but they can only function well and produce effective social value if they achieve a balanced development of medical and health care.While people are sharing the fruits of economic development,there has been no significant increase in satisfaction with maternal and child health institutions,and the development of maternal and child health institutions is in a dilemma.The reason for this is that there are problems in the operation and management of maternal and child health institutions.This paper uses the governance paradigm of public values to examine the problems in the mission,legitimacy and operational capacity of maternal and child health institutions,reorient the operation and management of maternal and child health institutions,and explore measures to return municipal maternal and child health institutions to their original mission and fulfill their positioning.This paper understands the operation and management of maternal and child health care institutions through literature research,and sorts out the relevant theories,methods,and dimensions of operation and management research of maternal and child health care institutions.Through field research,we collected qualitative and quantitative data on the operation and management situation,development trend,and policy environment of maternal and child health care institutions by using a case study method with W Maternal and Child Health Center as the research target.With the help of an analytical framework based on public value theory,we analyzed the problems in the operation and management of W Maternal and Child Health Center.The study found that public value conflicts existed in the mission management of municipal maternal and child health institutions.The financial pressure faced by the institution affected the achievement of its sustainable development goals.There is a large gap between the institution’s actual service capacity and the high requirements of its mission orientation.There is a lack of public responsibility in the political management of maternal and child health institutions.The government’s nature as an "Economist" determines its instinct to expand its power and avoid its obligations.The high expectations of the citizenry and the government’s transfer of conflicts have led to a weakening of the support and legitimacy of the MCH institutions.The internal operation and management of maternal and child health care institutions suffer from a lack of comprehensive capacity,which is manifested in the incomplete construction of operation and management systems,the lagging development of human resources and disciplines,and the poor internal two-way communication mechanism.To solve the problems in the operation and management of municipal maternal and child health care centers,we need to "work both inside and outside".It is necessary to guide municipal maternal and child health institutions to return to their missions,strengthen the support of multiple parties for the development of municipal maternal and child health institutions,and improve the operation and management capacity of municipal maternal and child health institutions.We need to break down the barriers between departments with the help of operation and management tools,and open up the "two veins" of the institution,so as to realize the transformation of the institution’s high-quality development. |
