Effect Study Of Clavicular Hook Plate Combined With Suture Anchors Fixation And The Application Of Clavicle Hook Plate In The Treatment Of NeerⅡb Type Discal Clavicle Fractures Under The Theory Of “ Emphasizing Both Muscles And Bones”

Objective Now,NeerⅡb type distal clavicle fractures can be fixed through various method and fixation of ligamenta coracoclaviculare is also a great concern.Thus,the choice of surgical methods in clinical treatment has become a big challenge for surgeons.Focus on comparison of the effect of clavicular hook plate combined with suture anchors fixation under the guidance of the theory of “emphasizing both muscles and bones” and the application of clavicle hook plate in the treatment of NeerⅡb type distal clavicle fractures.Hope it would provide a certain reference for the selection of surgical method in clinical treatment.Method A collection of patients with NeerⅡb distal clavicle fractures admitted to The Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2018 to March 2021 and a total of 50 cases were obtained.According to different internal fixation,they were divided into two groups: the clavicular hook plate combined with suture anchors fixation group and hook plate group(fixed with a clavicular hook plate only).25 cases were obtained in each group.A series of cases observation was collected: the preoperative coracoclavicular distance,postoperative coracoclavicular distance after a month and a year,the visual analog score in different period after surgery,Constant-Murley score,fracture healing time and complications.Then making statistical analysis to compare the difference in curative effect of the two groups of surgical methods.Results Patients in both groups achieved clinical healing.In hook plate group(fixed with a clavicular hook plate only),One patient appeared to have delayed incision healing but smoothly recovered after treatment.4 cases showed subacromial impingement syndrome.Another patient appeared to have subcromial osteolysis but the internal fixation still in good position.In the clavicular hook plate combined with suture anchors fixation group,2 patient suffer from subcromial impingement syndrome.There was no statistical significance between the two groups in terms of postoperative coracoclavicular distance and fracture healing time(P>0.05).For the visual analog score and Constant-Murley score in 1 month,3 months and 12 months after the surgery,the clavicular hook plate combined with suture anchors fixation was more dominant(P<0.05).There was a statistical significance between the two groups in terms of coracoclavicular of preoperative and postoperative performance in 1 month and in 1 year.(P<0.05)but no statistical difference(P>0.05).In terms of fracture healing times,there was no statistical significance in comparison for both groups(P>0.05).Conclusion For the treatment of patients with NeerⅡb distal clavicle fracture,both clavicle hook plate combined with suture anchors and application of clavicle hook plate are reliable.But the former has better clinical performance and can be recommended in clinical function.
