Objective:1.To analyze the risk factors related to venous thromboembolism(VTE) after breast cancer surgery, strengthen the understanding of VTE,an d reduce the incidence of VTE in patients undergoing breast cancer su rgery.2.To evaluate the efficacy and safety of Sichong Tablets in the trea tment of VTE and observe its clinical efficacy.Method:Part 1:A total of 48 patients with venous thromboembolism after breast cancer who were hospitalized at the Department of Breast Thyroid Surgery of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2020 to December 2021 were collected, then Randomly collected 60 patients with breast cancer of non-VTE after surgery during the same period as the control group. The clinical data of the patients, including age, menopausal status, height and weight, underlying diseases, coagulation indicators, surgical methods, surgical duration, pathological stages and molecular typing, were required to be complete. An Excel summary table was established, and SPSS23.0 software was used for statistical processing of all data to analyze the risk factors related to VTE after breast cancer surgery.Part 2:Forty-eight patients with VTE were randomly divided into experimental group and control group. On the basis of postoperative routine treatment, the experimental group was additionally given Sichong Tablets, and compared the changes of plasma fibrinogen(FIB),plasma D-dimer(D-Dimer),lower extremity venous Doppler ultrasound and venous thrombosis recirculation rate of the two groups. The adverse reactions such as allergy and damage of liver and kidney function were observed during the medication period, using SPSS23.0 data processing and draw a conclusion.Result:Part 1:(1)In the thrombosis group, there were 30 patients aged ≥60 years, accounting for the largest proportion(62.50%).The highest rate of thrombosis was found on the third day after surgery.Deep vein thrombosis of the right lower limb was the most common, and the proportion of intermuscular vein thrombosis was the highest.(2)There were no significant differences in surgical methods, pathological stages and molecular typing between the thrombus group and the control group(P<0.05).There were statistically significant differences in age, menopausal status, hypertension, obesity, operation duration,FIB and D-DImer(P<0.05).(3)Hypertension,obesity,FIB≥4.085g/L and D-Dimer≥0.91ug/ml were independent risk factors for VTE after breast cancer surgery(P<0.05).Part 2:After treatment,FIB level and D-Dimer level of experimental group and control group decreased significantly compared with before treatment(P<0.01).There were statistically significant differences in FIB level,D-Dimer level and venous recanalization rate between the experimental group and the control group(P<0.05).These results suggest that the two schemes are effective in the treatment of postoperative VTE of breast cancer, and the effect is more significant in the experimental group.Conclusion:1.The Obesity,hypertension,FIB≥4.085g/L and D-Dimer≥0.91ug/ml were all independent risk factors for postoperative VTE in breast cancer patients.2.The age, menopause, and operation duration > 4 hours were not independent risk factors for VTE after breast cancer surgery, but also had certain effect on the occurrence of VTE after breast cancer surgery. The incidence of VTE in elderly patients was significantly higher than that in young and middle-aged patients.3.Compared with conventional anticoagulant therapy, the addition of Sichong Tablets can better reduce the levels of plasma FIB and D-dimer in patients with VTE,reduce blood hypercoagulability, improve venous recanalization rate, and improve clinical efficacy with high safety.
