| Objective: on the one hand,through the investigation of syndrome elements in patients with hyperuricemia,this study preliminarily discusses the distribution law of TCM syndrome elements of hyperuricemia,so as to provide reference for clinical syndrome differentiation and treatment.On the other hand,based on the syndrome differentiation and treatment of traditional Chinese medicine and the combination of "disease syndrome elements" as the starting point,observe the clinical efficacy and safety of "sanghu uric acid Granule" in the treatment of hyperuricemia(phlegm dampness type)by the agreement Party of the major breakthrough project of Shandong Health Commission.Methods: 120 patients with hyperuricemia from the Second Affiliated Hospital of Shandong University of traditional Chinese medicine were included.The general demographic information,living habits,combined diseases and four diagnostic information of traditional Chinese medicine were recorded by questionnaire.62 patients who met the inclusion criteria of phlegm dampness type were included.The selected cases were randomly divided into 31 cases in the experimental group and 31 cases in the control group by simple randomized grouping design and coding according to the inclusion time sequence.During the experiment,2 cases fell off,and 30 cases in the experimental group and 30 cases in the control group were finally completed.The patients in both groups were given health education and lifestyle intervention,such as low purine diet,control of total calories intake,regular exercise and alkalization of urine with sodium bicarbonate when necessary.On this basis,the experimental group was given the traditional Chinese medicine "sanghuuric acid granules",The biochemical indexes measured before and after the experiment,serum uric acid,total cholesterol,triglyceride,liver and kidney function,blood and urine routine and TCM syndrome score were recorded,and the data were statistically processed.Results:(1)a total of 120 patients with hyperuricemia were collected,including 77 males and 43 females,with a male to female ratio of 1.79:1.(2)Symptom distribution: heavy body sleepiness(72.50%),head covering(65.83%),elevated blood glucose(63.33%),sticky mouth(50.83%),fatigue(45.00%),pharyngeal foreign body sensation(39.17%),high triglyceride(38.33%),obesity(37.50%),serious illness after activity fatigue(36.67%),thirst(35.83%)and other symptoms are common symptoms;Greasy tongue coating(42.50%),tooth mark on the edge of the tongue(39.17%)and light tongue(26.67%)were common tongue images;Slippery pulse(51.67%),wet pulse(39.17%)and the number of pulses(27.50%)were the most common.(3)Disease syndrome elements: from more to less,the order was dampness in 89 cases(74.17%),phlegm in 83 cases(69.17%),Qi deficiency in 68 cases(56.67%),qi stagnation in 55 cases(45.83%),blood stasis in 41 cases(34.17%),Yang deficiency in26 cases(21.67%).(4)Four diseases(40.00%)were the most common combination of disease syndrome elements,followed by three diseases(33.33%)and two diseases(24.17%);The specific manifestations are dampness,phlegm,qi stagnation,Qi deficiency(15.00%),dampness,phlegm,Qi deficiency,blood stasis(13.33%),dampness,phlegm and Qi Stagnation(13.33%).(5)Sanghu uric acid granule can improve the level of blood uric acid in patients with phlegm dampness type hyperuricemia.After 8 weeks of treatment,the SUA of the two groups decreased in varying degrees.After treatment,the SUA of the experimental group was lower than that of the control group,and the decrease of SUA level in the experimental group was significantly higher than that in the control group(P < 0.01).(6)Sanghuuric acid granule can significantly improve the total score of TCM syndromes in patients with phlegm dampness combined hyperuricemia.After 8 weeks of treatment,the total score of TCM syndromes in the two groups was lower than that in the previous group,and the experimental group was significantly lower than that in the control group(P < 0.01);The total effective rate of traditional Chinese medicine in the experimental group was 93.33% and that in the control group was 56.67%,suggesting that the curative effect of the experimental group was significantly better than that of the control group(P < 0.01).(7)Sanghuuric acid granule can significantly improve the fasting blood glucose and the levels of TG,LDL-C and TC in patients with phlegm dampness combined hyperuricemia(P < 0.01).(8)There were no significant adverse reactions in the two groups,and there was no significant difference in the safety indexes alt,AST,bun,Cr and blood routine before and after treatment(P > 0.05),suggesting good safety.Conclusion: 1 The TCM syndrome elements of hyperuricemia are mainly dampness,phlegm and Qi deficiency.2.Sanghu uric acid granule can significantly reduce the level of serum uric acid in patients with hyperuricemia and significantly improve the metabolic indexes of combined diseases.It is safe to use and has the characteristics of multi-target,safe and effective in treatment. |
