Objective: The purpose of this study is to observe the clinical efficacy of Tongguan Zhilin Decoction in the treatment of urinary tract infection in middle-aged and elderly women with kidney yin deficiency and bladder damp heat,and to explore the feasibility and safety of Tongguan Zhilin Decoction in the treatment of urinary tract infection in middle-aged and elderly women,so as to provide new ideas and basis for the clinical application of traditional Chinese medicine in the treatment of urinary tract infection in middle-aged and elderly women.Methods: The 70 patients who met the inclusion criteria were randomly divided into control group and experimental group,with 35 cases in each group.Both groups were given basic treatment.The control group was given furantoin tablets orally,and the antibiotic treatment scheme was maintained or adjusted according to the results of urinary culture and drug sensitivity.The experimental group was treated with Tongguan Zhilin Decoction on the basis of the control group.The course of treatment was 14 days and followed up for 3 months.Laboratory indexes and TCM syndrome quantitative scores of the two groups before and after treatment were collected and recorded,and recurrence were followed up.The data of the two groups were statistically analyzed to determine the clinical efficacy and safety.Results:A total of 66 patients completed the study,34 in the experimental group and32 in the control group.(1)Clinical comprehensive efficacy:The total clinical effective rate of the experimental group was 91.18%,while that of the control group was 68.75%.The difference in clinical comprehensive efficacy between the two groups was statistically significant(P<0.05).(2)Efficacy of TCM syndromes:The effective rate of TCM Syndrome of the experimental group was 94%,and that of the control group was 75%.The difference in efficacy of TCM syndromes between the two groups was statistically significant(P<0.05).(3)TCM syndromes:(1)Main symptoms:Except for abdominal discomfort(P>0.05),the differences in frequency of urination,urgency of urination,pain of urination and burning of urination were statistically significant(P<0.05).(2)Secondary symptoms:There were significant differences in waist and knee soreness,dysphoria in chestpalms-soles,dry mouth and pharynx,fatigue,hot flashes and night sweats(P<0.05).(4)Laboratory indicators:(1)Routine urinalysis: After treatment,there were statistically significant differences in the number of white biood and red blood cells in urine between the two groups(P<0.05).After treatment,The positive rate of nitrite in the two groups decreased compared with that before treatment,but there was no significant difference(P>0.05).(2)Urine bacterial culture:There was statistically significant difference in urine culture results between the two groups after treatment(P<0.05).The difference of urinary bacteriological efficacy was statistically significant(P<0.05).(5)Recurrence rate: the recurrence rate of the experimental group was 8.82% and that of the control group was 31.25%.There was statistically significant difference between the two groups(P<0.05).(6)Safety: The safety indexes of the two groups were not obvious abnormal.Conclusions: Compared with simple antibiotic treatment,Tongguan Zhilin decoction combined with antibiotics in the treatment of middle-aged and elderly women with urinary tract infection can not only improve laboratory indicators,enhance anti-inflammatory and bacteriostatic effect,but also alleviate clinical symptoms and improve quality of life.Moreover,it has safe medication and low recurrence rate.It is beneficial to long-term curative effect and is worthy of clinical promotion.
