| ObjectiveThe purpose of this study was to construct the core competency evaluation index system for undergraduate nursing interns and test the reliability and validity of the evaluation index system to verify its effectiveness and feasibility and provide a practical basis for clinical nursing practice assessment,evaluation,and hospital selection and recruitment of excellent undergraduate nursing talents.Methods1.Behavioral event interview.Objective sampling was used to select 20 undergraduate nursing interns with excellent performance and two groups of ordinary undergraduate nursing interns to conduct behavioral event interviews.The interviewed nursing students were asked to describe in detail the three successful or failed events in the course of clinical nursing practice.The core competency of nursing undergraduate interns was summarized by content analysis method,and the evaluation index system of core competency of nursing undergraduate interns was preliminarily formulated.2.Delphi expert consultation and analytic hierarchy process.Twenty experts from seven provinces and cities in China were selected to screen,modify and improve the preliminary indicators of the core competency evaluation index system of nursing undergraduate interns through two rounds of Delphi expert consultation.The weight of each index was calculated by the analytic hierarchy process,and the core competency evaluation index system of nursing undergraduate interns and the weight of primary and secondary indicators were determined.3.Questionnaire survey method.The core competency self-assessment questionnaire of nursing undergraduate interns based on the evaluation index system was used as a tool to investigate 302 nursing students who met the inclusion and exclusion criteria.The mean and standard deviation were used to represent the current score of core competency of nursing undergraduate interns.The independent sample t test and single factor analysis of variance were used to compare the scores of different demographic data.At the same time,the critical ratio method and the total analysis method were used to analyze the project,the internal consistency coefficient was used to analyze the reliability,and the expert evaluation,correlation analysis and exploratory factor analysis were used to analyze the content validity and structural validity to test the internal quality of the index system.Results1.Through behavioral event interview,30 nursing undergraduate interns’ core competency entries were summarized.The total interview time was 1116.31 minutes,the total number of interviews was 236715 words,and the coding classification consistency coefficient was 0.80~0.97.Combined with previous literature research and competency onion model,the core competency evaluation index system for undergraduate nursing interns was preliminarily constructed,which included eight first-level indicators of professional theoretical knowledge,clinical nursing ability,professional attitude,professional values,role cognition,self-image,personality traits,psychological motivation,and 30 second-level indicators and their connotations.2.Through the Delphi expert consultation method,the evaluation index system of the core competency of nursing interns is finally constructed,including 7 first-level indicators,28 second-level indicators,and connotations.The positive coefficient of the two rounds of the Delphi method is 80%and 100%respectively,and the authority coefficient is 0.743 and 0.788 respectively.The index variation coefficient of the two rounds of expert consultation is between 0.143 and 0.167.The difference in the significance level test of the Kendall harmony coefficient is statistically significant(P<0.01),and the degree of coordination of expert opinions is good.3.Through the analytic hierarchy process,the first-level index weight scores from high to low are professional ability 0.3062,professional knowledge and skills 0.2528,personality trait 0.1622,psychological motivation 0.0928,professional values 0.0811,professional attitude 0.0680,self-concept 0.0369.The top four in the order of weight value of the secondary index were nursing operation skills 0.1444,interpersonal communication and cooperation 0.0892,critical thinking ability 0.0707,and nursing theoretical knowledge 0.0666.4.The result of item analysis of the evaluation index system of the core competency of undergraduate nursing interns showed that the score of each index item in the high score group was significantly higher than that in the low score group,and the t value was between 9.358 and 17.173(t>3,P<0.001).The correlation coefficient between each item score and the total score was 0.566-0.744(P<0.001).The overall Cronbach’s α coefficient of the evaluation index system is 0.945,and the internal consistency reliability is good;the average S-CVI value of the evaluation index system is 0.989,and the content validity is good;the KMO value of exploratory factor analysis is 0.943 and the spherical test value of Bartlett’s is 4608.117(P<0.001).The cumulative variance contribution rate of the seven common factors extracted is 69.26%,and the structure validity is good.5.The results of the questionnaire survey showed that the total score of the core competency of 302 undergraduate nursing interns was(96.91±19.58),and the results of the univariate analysis showed that there were significant differences in attitude towards nursing major,academic achievement during the undergraduate course and current clinical practice stage.Conclusion1.Through literature analysis,behavioral event interviews,Delphi expert consultation,and a hierarchical analysis process,this study finally constructed a core competency evaluation index system for undergraduate nursing interns containing 7 primary indicators,28 secondary indicators,and connotations,and set corresponding weight coefficients for indicators at all levels with focused expert opinions,comprehensive content and reasonable structure.2.The core competency evaluation index system constructed in this study has good reliability and validity,which can provide a scientific reference basis for core competency evaluation and training of undergraduate nursing interns,and is important for clinical nursing practice assessment as well as recruitment,selection,and training of excellent nursing talents.3.Through the investigation,it is found that the core competency of undergraduate nursing interns is in the middle level,and the attitude of nursing students towards nursing major,their academic achievements during the undergraduate course,and the current stage of clinical practice will affect the level of their core competency.Nursing educators need to take corresponding measures to cultivate the core competency of undergraduate nursing interns. |
