| Meta-analysis and Systematic Evaluation of Combination of Dotted and Solid Thread-Ligating Therapy in the Treatment of High Anal FistulaObjective:Systematic evaluation of the clinical effect of treating high anal fistula with combination of dotted and solid thread-ligating therapy and traditional solid thread-ligating therapy.Methods:Computer search the CNKI,WANFANG,CBM,and CQVIP.The search time is from the establishment of the database to January 2021,screening We published all clinical randomized controlled trails(RCTs)using the combination of dotted and solid thread-ligating therapy in the treatment of high anal fistulas.At the same time,we manually searched the "Chinese Journal of Anorectal Diseases","Colorectal and Anal Surgery".Complete literature search,screening and literature quality evaluation,and extract relevant data.The RevMan 5.3 software was used to merge and analyze the outcome indicators.Results:Meta analysis results showed that the total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant[OR=10.53,95%CI(5.35,20.71),Z=6.81(P<0.0000];postoperative wound healing time in the treatment group[MD=-8.31,95%CI(-10.68,-5.94),Z=6.87(P<0.00001)],pain score on the first day after surgery[MD=-1.83,95%CI(-2.68,-0.99),Z=4.26(P<0.0001)],pain score on the seventh day after surgery[MD=-1.65,95%CI(-2.49,-0.81),Z=3.85(P=0.0001)],analincontinence Wexner evaluation[MD=-2.61,95%CI(-3.08,-2.14),Z=10.87(P<0.00001)].All are lower than the control group,and the differences are statistically significant.Conclusion:The combination of dotted and solid thread-ligating therapy method has a better clinical effect in the treatment of high anal fistulas.Compared with the traditional thread-ligating therapy method,thepostoperative wound healing time is shorter,the postoperative pain is low,and the anal sphincter function can be protected to a greater extent.Clinical Research on the Treatment of High Anal Fistula With Combination of Dotted and Solid Thread-Ligating Therapy by Traditional Chinese Medicine Medicated ThreadObjective:To observe and evaluate the clinical efficacy andadvantages of the combination of dotted and solid thread-ligating therapy of thetraditional Chinese medicine medicated thread in the treatment of high anal fistula.Methods:From April 2020 to December 2020,44 inpatients in Department of Anorectal Medicine of Affiliated Hospital of North Sichuan Medical Collegeour who met the diagnosis of high anal fistula were selected and divided into observation group(n=22)and control group(n=22)according to the random number table.The observation group was treated with combination of dotted and solid thread-ligating therapy by traditional Chinese medicine medicated thread,and the control group was treated with rubber band real hanging and rubber strip virtual hanging method.The total effective rate after treatment,the degree of pain at different time points after the operation,the shape of wound granulation tissue,the amount of wound secretion exudation,wound healing time,and anal function were observed in the two groups.Results:After statistical analysis,there was no statistically significant difference in pain VAS scores between the two groups on the 3rd and 14th day after surgery(P>0.05),while the difference in pain VAS scores between the two groups on the 7th day after surgery was statistically significant(P<0.05),indicating that the observation group is better than the control group;the difference in wound granulation tissue morphology scores between the two groups on the 7,14 and 21 days after surgery is statistically significant(P<0.05),indicating that the observation group is better than the control group;There was no statistically significant difference in wound secretion exudation score between the two groups on the 3rd and 14th day(P>0.05),and the difference in wound secretion exudation score between the two groups on the 7th day after surgery was statistically significant(P<0.05),indicating that the observation group is better than the control group;the difference in wound healing time between the two groups is statistically significant(P<0.05),indicating that the observation group is better than the control group;after recovery,the difference in Wexner scores of anal incontinence between the two groups is statistically significant(P<0.05),indicating that the observation group is better than the control group.Conclusion:The combination of dotted and solid thread-ligating therapy of the traditional Chinese medicine medicated thread in the treatment of high anal fistula embodies the "one-line multi-purpose".It has a definite clinical effect.It can promote secretion drainage and granulation tissue growth,promote wound healing,and reduce postoperative pain in patients.It protects the anal sphincter function and is worthy of further clinical research and promotion. |
