Effect Of Shujin Huoxue Decoction Combined With Rehabilitation Exercise On Early Postoperative Shoulder Joint Function Of Proximal Humeral Two-part Fracture

Objective:To investigate the effect of Shujin Huoxue Decoction combined with rehabilitation exercise on early postoperative shoulder joint function of proximal humeral two-part fracture.Method:A total of 62 patients with qi stagnation and blood stasis combined with deficiency of liver and kidney were selected from the department of Orthopaedics of our hospital from January 2021 to December 2021 after locking plate internal fixation surgery for two-part proximal humeral fractures.A simple randomized grouping method was used to generate a random number by looking up a random number table,and a sequence numbered,opaque sealed envelope was used to preserve the randomly assigned treatment plan.The envelopes were opened according to the sequence of numbers on the envelopes,and 62 patients were randomly divided into experimental group(n = 31)and control group(n = 31)according to the intra-envelope allocation scheme.The changes of peri-shoulder diameter,visual analogue scale(VAS)and Neer score of shoulder were observed and compared between the experimental group and the control group before operation and on day 1,7,14 and 30 after operation.Results:A total of 60 patients completed the study,2 cases of detachment,30 cases in the experimental group and 30 cases in the control group.1.There was no significant difference in age,sex,weight,height and underlying diseases between the two groups(P>0.05).2.There was no statistically significant difference between the experimental group and the control group in the shoulder circumference difference of the healthy side and the affected side before and on the first day after surgery(P > 0.05).On the 7th,14 th and 30 th postoperative days,the difference of shoulder circumference between the healthy side and the affected side in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).3.There was no significant difference in VAS scores between the experimental group and the control group before and on the first day after surgery(P > 0.05).VAS scores of the experimental group on the 7th,14 th and 30 th postoperative days were lower than those of the control group,with statistically significant differences(P<0.05).4.There was no significant difference in Neer score of shoulder between the experimental group and the control group before and after surgery(P > 0.05).The Neer score of the experimental group was higher than that of the control group on the 7th,14 th and 30 th postoperative days,and the difference was statistically significant(P<0.05).The excellent and good rate of the experimental group was significantly higher than that of the control group on the 30 th day after operation,and the difference was statistically significant(P <0.05).5.There was no significant difference in TCM syndrome scores between the experimental group and the control group before treatment(P>0.05),after treatment,the TCM syndrome score of the experimental group was significantly lower than that of the control group,and the TCM syndrome efficacy was significantly better than that of the control group,the differences were statistically significant(P<0.05).6.Safety assessment: No allergic reaction,nausea,vomiting,abnormal liver and kidney functions were found in both groups during the treatment.Conclusion:Shujin Huoxue Decoction combined with rehabilitation exercise can effectively improve the early swelling,pain and range of motion of shoulder joint in postoperative patients with proximal humeral two-part fracture,promote functional rehabilitation and prevent shoulder stiffness.
