Research On Professor Xue Hanrong’s Medication Experience In Differentiating And Treating Bronchiectasis Based On Data Mining

Objective:To summarize professor Xue Hanrong’s experience and rule of treating bronchiectasis by analyzing medical cases through data mining.Methods: By collecting and sorting out medical case analysis,combining with reading relevant literature,clinical practice and teacher learning,this paper summarizes professor Xue Hanrong’s experience in the differentiation and treatment of bronchiectasis.The medical records were then sorted out and imported into the Traditional Chinese Medicine Inheritance Auxiliary Platform(V2.5),and the collected medical records were statistically analyzed through the "Statistical" and "Data Analysis" functions of the system,so as to summarize the medication rules of the professor.Results:Preliminary summary Xue Hanrong professor’s experience in the differentiation and treatment of bronchiectasis,think bronchiectasis pathogenesis is always " deficiency in origin and excess in superficiality," this deficiency in origin is dual deficiency of spleen and lung,that excess in superficiality is associated with cold pathogen,warm pathogen,phlegm,dampness pathogen,stagnated blood,other exess pathogen,and so on,which is mainly mixture of asthenia and sthenia,summarized commonly used treatment methods and clinical experience,as well as the treatment of 11 syndrome types,such as pattern of external cold and internal retention of fluid,syndrome of exterior cold and interior heat,syndrome of pathogenic heat obstructing the lung,syndrome of phlegm and heat congesting in the lung,syndrome of phlegm and heat and stagnation in the lung,syndrome of phlegm and heat damaging yin,syndrome of phlegm and heat harassing the internal,syndrome of liver fire invading the lung,syndrome of dampness and heat in the lung meridian,pattern of dual deficiency of spleen and lung,syndrome of qi and yang deficiency.Through data mining,medical case data of 287 medical records are analyzed as follows:1.In the statistics of syndromes,in the acute exacerbation of bronchiectasis,the frequency of syndrome of pathogenic heat obstructing the lung was the highest,followed by syndrome of exterior cold and interior heat,syndrome of phlegm and heat damaging yin,syndrome of phlegm and heat congesting in the lung,syndrome of phlegm and heat and stagnation in the lung,syndrome of phlegm and heat harassing the internal.In the stable stage,the frequency of syndrome of deficiency of spleen and lung was the highest.Separate statistics on the syndrome of bronchiectasis with hemoptysis,the frequency of syndrome of phlegm and heat congesting in the lung was found to be the highest.In the symptom statistics,in the acute exacerbation,the frequency of cough was the highest,followed by dry mouth,chest tightness,fatigue,asthma,aversion to cold,and yellow sputum;in the stable stage,the frequency of cough was the highest,followed by fatigue,etc.In the statistics of tongue and pulse,in the acute exacerbation,the proportion of red tongue,yellow coating,greasy coating,string pulse and slippery pulse is relatively high;In the stable stage,the proportion of pale red tongue,white coating,thin pulse,string pulse and slip pulse is heavier.In the statistics of herbs frequency,in the acute exacerbation,the frequency of ephedra is the highest;in the stable period,the frequency of Atractylodes is the highest;The most commonly used hemostatic herbs are Sophorae Flos,Agrimoniae Herba,and Imperatae Rhizoma.2.Chinese herbal medicines used in the acute exacerbation stage: among the four properties,the proportion of warm herbs and cold herbs was close to 1:1,followed by neutral herbs,among the five flavors,bitter-taste,spicy-taste and sweet-taste accounted for the largest proportion,with the proportion of nearly 1:1:1,among the channel tropism,most drugs went to the lung,spleen and stomach channels,followed by the heart,large intestine,liver and kidney channels;Chinese herbal medicines used in the stable stage,among the four properties,warm herbs were the majority,followed by neutral herbs and cold herbs,among the five flavors,sweet herbs were the most,followed by bitter herbs and spicy herbs,among the channel tropism,most of the herbs belonged to lung and spleen channels,followed by stomach,kidney,heart and liver channels.3.In the association rules analysis,the overall prescription rules analysis showed that the main herbs were Ephedra,Almond,Gypsum,Glycyrrhiza,prepared Glycyrrhiza,Platycodon Grandiflorum,Tangerine Peel,Pinelliae Rhizoma,Tuckahoe,Radix Scutellariae,Mori Cortex,Houttuynia Herba,Golden Buckwheat and Ginger in the acute exacerbation stage,and the main herbs were Radix Codonopsis,Atractylodes,Poria cocos,Yam,Platycodon Grandiflorum,Coix Seed and Tangerine Peel in the stable stage.And analyzed the main main herbs of main syndrome,in the acute exacerbation stage,the main herbs of pathogenic heat obstructing lung syndrome are Ephedra,Almond and Gypsum;the main herbs of exterior cold and interior heat syndrome are Ephedra,Pinelliae Rhizoma,cassia Twig,Asarum,Prepared Glycyrrhiza,Gypsum,Rhizoma Zingiberis,Almond,White Peony Root,Schisandra Fruit,Perilla Fruit and Scutellaria baicalensis;the main herbs of phlegm and heat damaging yin syndrome are Ephedra,Almond,Poria cocos,Mori Cortex,Tangerine Peel,Gypsum,Gardenia,Trichosanthes Semen,Ophiopogon,Fritillaria Thunbergii,Scutellaria Baicalensis,Platycodon Grandiflorum,Rhizoma Anemarrhenae and Glycyrrhiza;the main herbs of phlegm and heat congesting in the lung syndrome are Tangerine Peel,Rhizoma Cholangata,Gypsum,Pinelliae Rhizoma,Ephedra,Almond,Trichosanthes Semen,Golden Buckwheat,Houttuynia Herba,Tuckahoe,Aurantii Frutus Immaturus and Scutellaria Baicalensis;the main herbs of phlegm and heat and stagnation in the lung syndrome are Tangerine Peel,Scutellaria Baicalensis,Golden Buckwheat,Houttuynia Herba,Aconite,Coix Seed,Patriniae herba,Almond,Ephedra,Phragmitis Rhizoma,Glycyrrhiza,Persicae Semen,Benincasae Semen and Gypsum.the main herbs of phlegm and heat harassing the internal syndrome are Scutellaria Baicalensis,Bambusae Caulis in Taenias,Ginger,Pinelliae Rhizoma,Tuckahoe,Prepared Glycyrrhiza,Jujubae Fructus.In the stable stage,the main herbs of dual deficiency of spleen and lung syndrome are Dolicho Lablabl,Coix Seed,Amomum villosum Lour,Yam,Atractylodes,Radix Codonopsis,Platycodon Grandiflorum,Nelumbinis Plumula,Poria cocos and Tangerine Peel.4.In the analysis of new prescriptions,28 new prescriptions were obtained in acute exacerbation and 12 new prescriptions were obtained in the stable stage.Conclusion:The pathogenesis of bronchiectasis always belongs to " deficiency in origin and excess in superficiality" and "mixture of asthenia and sthenia".In the stable stage,the main pathogenesis is deficiency in origin,and the main deficiency is lung and spleen,which is often concurrently mixed with external pathogenic factors or heat stagnation.In the acute exacerbation stage,the main pathogenesis is excess in superficiality,which is mostly associated with cold pathogen,warm pathogen,phlegm,dampness pathogen,stagnated blood,other exess pathogen,and so on.In terms of treatment,methods such as warming lung,clearing heat,resolving phlegm,diffusing the lung to downbear counterflow and stopping bleeding are commonly used,the symptoms of acute exacerbation are relieved from multiple perspectives in the acute exacerbation;in the stable stage,mainly to strengthening healthy qi and tonification,commonly used method of tonifying the spleen and replenishing the lung,with more warm herbs.
