Objective:To observe the immediate analgesic effect and overall efficacy of giant acupuncture in the treatment of migraine without aura,and to explore the theoretical basis of giant acupuncture in the treatment of this disease.Methods:Sixty-two patients who met the inclusion criteria were divided into treatment group and control group using a random number table,with 31 patients in each group.The treatment group used the giant thorn method,acupuncture on the normal side of Taiyang,Sizhukong,Shuaigu,Fengchi,Waiguan,Xiaxi.The control group was treated with conventional acupuncture,and the same acupuncture points were taken from the affected side.In the acute attack stage,after the Qi was obtained by acupuncture,reducing by lifting and thrusting or rotating needles was used for 5min,and then the needle was retained for 25min.In the remission period,acupuncture at each acupoint was performed for 5 minutes after Qi was obtained,and the needle was kept for 25 minutes.It was treated once a day for 30min each time.One week is a course of treatment,6 days for treatment,1 day for rest,a total of 4 courses of treatment.Visual Analogue Score(VAS)was performed before treatment and 0.5h,1 h,and 2h after initial treatment to evaluate the pain degree.The headache scale was performed before and after treatment to evaluate the improvement of symptoms and the overall clinical efficacy.A migraine specific quality of life questionnaire(MSQ)score was performed before and after treatment to evaluate the improvement of quality of life before and after treatment.SPSS26.0 was used for statistical analysis of the collected data to judge the clinical efficacy.Results:1.After 0.5h,1 h and 2h of initial acupuncture treatment,VAS scores in both groups were lower than those before treatment,with statistical significance(P<0.05).At 0.5h and 1 h after the initial treatment,the comparison between groups was statistically significant(P<0.01).2 hours after treatment,there was no significant difference between the two groups(P>0.05).2.After treatment,compared with before treatment,the number of headache attacks,headache severity,duration,accompanying symptoms and total scores in the headache rating scale of the two groups were significantly improved,and statistically significant differences were found(P<0.01).Compared between groups,the therapeutic effect of treatment group was better than control group in improving symptoms and total scores(P<0.05).3.After treatment,MSQ scores of all dimensions in both groups increased compared with before treatment,with statistically significant difference(P<0.01).There was significant difference between the two groups in the dimension of role limitation and role hindrance(P<0.01).In the dimension of emotional function,there was no statistical significance between the two groups(P>0.05).4.The total effective rate was 93.55%in the treatment group and 80.65%in the control group,the difference was statistically significant(P<0.01).Conclusion:1.Both conventional acupuncture method and giant thorn method can alleviate the headache degree of migraine patients,but the giant thorn method has more significant analgesic effect at 0.5h and 1 h after the initial treatment,and the immediate analgesia effect is more prominent.2.Giant thorn method has more obvious efficacy in improving the frequency of migraine attack,headache degree,duration and accompanying symptoms.3.Giant thorn therapy has a more prominent therapeutic effect in improving the normal function limitation degree and the normal life disturbance degree of migraine patients.In the improvement of emotional function,the effect of giant acupuncture was similar to that of conventional acupuncture.
