| Objective:To observe the clinical distribution characteristics of "fullness of abdominal distension" in children with anorexia, verify the practicality and scientificity of the theory of abdominal diagnosis in traditional Chinese medicine, and enrich the connotation of spleen stomach disease.Methods:225 children with anorexia diagnosed in the pediatric clinic of the First Affiliated Hospital of Heilongjiang University of traditional Chinese medicine from December 2020 to March 2022 were taken as the research objects.Using the epidemiological cross-sectional investigation and research method, two doctors conducted TCM syndrome differentiation and abdominal diagnosis on anorexia children respectively. The researchers used a unified situation questionnaire to record the general situation, syndrome type and abdominal diagnosis of children,The children were divided into groups according to syndrome types.Main outcome measures: appetite score, frequency, location and severity of abdominal syndrome in different syndrome types.Result:1.Among the four syndrome groups of 225 children with anorexia, the spleen stomach qi deficiency group had 38 cases of moderate abdominal distention,10 cases of excessive abdominal distention and 4 cases of excessive abdominal distention, accounting for 55.9%,14.7% and 5.9%respectively in the syndrome group. The abnormal rate of abdominal fullness in the spleen stomach qi deficiency group was significantly higher than that in the other four groups(P< 0.05).2.In the distribution of 225 anorexia syndrome types,116 cases(51.6%)had spleen dysfunction syndrome,68 cases(30.2%)had spleen stomach qi deficiency syndrome,32 cases(14.2%)had liver hyperactivity and spleen deficiency syndrome, and 9 cases(4.0%)had spleen stomach yin deficiency syndrome.3.Among 225 children with anorexia, the proportion of total abdominal distension in the abnormal abdominal distension of each syndrome type was82.8% in the spleen dysfunctional group,92.3% in the spleen stomach qi deficiency group,71.4% in the liver hyperactivity and spleen deficiency group and 66.7% in the spleen stomach yin deficiency group.4.Among 225 children with anorexia,31 cases were complicated with epigastric pain, with an average age of 8.06 ± 1.33 years.Conclusion:1.Anorexia is mainly characterized by spleen dysfunction syndrome in the distribution of syndrome types, and there are few other symptoms except anorexia in spleen dysfunction syndrome.2.Fullness of abdominal distension is correlated with anorexia spleen stomach qi deficiency syndrome, and fullness of abdominal distension cannot be proved to be correlated with anorexia spleen dysfunction syndrome.3.Abdominal fullness in spleen stomach qi deficiency syndrome is mainly total abdominal fullness, and epigastric pain mostly occurs in older children. |
