Clinical Observation On The Treatment Of Refractory Perianal Pruritus By Perianal Nerve Free Block

Objective:To observe the clinical effect of perianal subcutaneous nerve free block combined with traditional Chinese medicine in the treatment of intractable perianal pruritus.Methods:120 patients with Intractable perianal pruritus(IPP)who met the inclusion criteria were divided into three groups:treatment group(perianal subcutaneous nerve free blocking combined with Traditional Chinese medicine sitting bath,n=40),control group A(skin flap free blocking,n=40)and control group B(perianal subcutaneous nerve free blocking,n=40).Compare the difference of three groups of patients before and after treatment and the differences between groups,record the related clinical data,using spss25.0 statistical software for three groups of patients in post operative pain,it chy skin lesion degree,degree,anti-inflammatory and wound exudate,sleep quality,postoperative complications,the total cure rate statistics analysis,Finally to evaluate the clinical effect of three groups of patients.Results:(1)In terms of post operative pain,the pain in control group A was more obvious,followed by control group B,and the pain in treatment group was the least.(2)In terms of skin lesions,patients in the treatment group and control group B could see a longitudinal incision at 6 o’clock and 12 o’clock,with a length less than 2 cm.Patients in control group A had obvious fusiform skin defects.(3)In terms of pruritus: the anti-pruritus effect was significant in the three groups after operation,but the cure rate was lower in the control group A;(4)Anti-inflammatory: the post operative anti-inflammatory time in the treatment group was shorter,followed by the control group B and the control group A;(5)Exudation:The patients in control group A had more exudation,followed by the patients in control group B,and the patients in treatment group had less exudation;(6)Sleep quality:The sleep of the three groups of patients improved significantly after surgery,and there was no significant difference in sleep quality between the three groups of patients after surgery;(7)Perioperative complications: All patients in the three groups had different degrees of postoperative complications,but none of them had serious effects.(8)total cure rate:the total cure rate of the treatment group reached 100%,control group A 80% control group B 92.5%.The results showed that there was difference between perianal nerve free block and flap free block in the treatment of intractable perianal pruritus.Conclusion: Crissum skin free nerve block technique combined traditional Chinese medicine(TCM)method,sitz bath treatment crissum itching sitz bath of Chinese herbal medicine can relieve patients postoperative pain,crissum skin nerve blocking free surgery treatment effect is exact,over along period of time,a combination of existing moss samples change skin repair effect to a certain extent,has great value in clinical promotion.
