Objective: Experiment 1 explored the correlation between intestinal flora imbalance and normal defecation.Experiment 2 to explore the safety and clinical efficacy of probiotics and traditional Chinese medicine compound preparation in the prevention and treatment of complications after RPH of mixed hemorrhoids.Method: In this study,90 patients with mixed hemorrhoids were randomly selected from the Department of anus and Gastroenterology of the first affiliated Hospital of Tianjin University of traditional Chinese Medicine from September 2020 to December 2021.The first experiment was divided into two groups according to the results of preoperative stool culture, namely, the control group(normal intestinal flora group)and the case group(intestinal flora disorder group).Record the defecation of different groups of patients before admission: defecation frequency, defecation difficulty degree, frequency of defecation difficulty, time to go to the toilet each time, whether to assist defecation, stool characteristics and defecation insufficiency. The correlation between defecation habit and intestinal flora imbalance was analyzed. In experiment 2,the patients were randomly divided into control group and experimental group,in which the experimental group was divided into three groups: traditional Chinese medicine compound preparation group and probiotic preparation group, with 30 cases in each group. The preoperative examination and preparation, the mode of operation, routine treatment and nursing care were consistent in the three groups. On the first day after operation, the traditional Chinese medicine compound preparation group was given oral Zhixue granule, the probiotic preparation group was treated with Bifidobacterium triple live bacteria capsule, and the control group was given routine treatment.the clinical data related to postoperative anal pain, anal distension, first defecation time, stool characteristics, defecation difficulty and defecation bleeding in the three groups were recorded and analyzed. Results:Experiment 1: after screening by exclusion criteria and exclusion criteria, the stool culture results of 55 inpatients were included in the analysis of intestinal flora, including 18 cases of intestinal flora disorder(32.73%).Intestinal flora was normal in 37 cases(67.27%).Combined with the defecation of 55 patients before admission,by t-test or rank sum test,it was found that there was no significant difference in age, sex, times of defecation, degree of difficulty in defecation, frequency of difficulty in defecation, time to go to the toilet, assistance in defecation, stool characteristics, incomplete defecation and intestinal flora imbalance(P > 0.05).The results of binary Logistic regression analysis showed that there was no significant difference.Experiment 2:1 The clinical data of 90 patients with mixed hemorrhoids were included in this study. There was no significant difference in age, sex, course of disease and disease stage among the three groups(P > 0.05).2 There was no significant difference in anal pain 12 hours after operation,12 hours,24 hours,48 hours,72 hours, the first defecation time and the first defecation time among the three groups(P > 0.05).3 The anal pain and the first postoperative defecation bleeding in the three groups were compared at 24 hours,48 hours and 72 hours after operation(P < 0.05).It was found that there were significant differences between the traditional Chinese medicine compound preparation group, the probiotics preparation group and the control group(P < 0.05).4 The characteristics and difficulty of defecation for the first time after operation were compared among the three groups(P < 0.05).It was found that there were significant differences between the probiotics group and the traditional Chinese medicine compound preparation group and the control group(P < 0.05).Conclusion:1 There was no significant correlation between intestinal flora imbalance and normal defecation habits in patients with mixed hemorrhoids.2 Chinese medicine compound preparation Zhixue granule can relieve anal pain and reduce defecation bleeding after RPH of mixed hemorrhoids.3 Peifeikang,a probiotic preparation, can improve the characteristics of the first defecation after mixed hemorrhoids RPH,make the defecation smooth, reduce the impact on postoperative wound recovery caused by too dry or sparse stool, and reduce the defecation pain and psychological burden of patients.4 Clinical probiotics can be used to regulate the stool characteristics of patients with mixed hemorrhoids after RPH,assist patients with defecation, make patients defecate smoothly, and combined with traditional Chinese medicine compound preparation, more effectively relieve postoperative wound pain, and reduce the possibility of postoperative defecation bleeding, further achieve the treatment and prevention of postoperative complications of mixed hemorrhoids RPH,and reduce the pain of patients.
