Objective: By observing and comparing the clinical curative effects of acupuncture combined with Kegel training at different depths on female stress urinary incontinence,this study verifies the effectiveness and safety of acupuncture combined with Kegel training on female stress urinary incontinence,explores the advantages of elongated needling therapy,and provides relevant clinical data of acupuncture on female stress urinary incontinence,in order to contribute new ideas and acupuncture schemes for the treatment of female stress urinary incontinence.Method: Seventy-two female patients with stress urinary incontinence who met the requirements of this study were randomly divided into the elongated needle groupand the filiform needle group according to the ratio of 1: 1.Both groups were treated with basic therapy,Kegel training and acupuncture.The acupuncture points of the two groups are:Zhongwan,Qihai,Guanyuan,Zhongji and points on both sides of Shuidao,Guilai,Zusanli,Yinlingquan,Sanyinjiao and Taixi.In the Mang acupuncture group,0.25×100mm disposable acupuncture needle was used for deep needling when needling Qihai,Guanyuan,Zhongji,Shuidao(double)and Guilai(double),and other acupuncture points were all conventional needling.All acupuncture points in the filiform needle group were treated with routine acupuncture.Everyone had the acupuncture once a day,six times a week.2 weeks is a course of treatment.The improvement of urinary incontinence in the two groups was observed before and after treatment.The 24-hour urinary incontinence times,ICI-Q-SF,I-QOL and SDS were used as observation indexes.We set up the database,sort out the research data of effective cases and analyzed statistic by SPSS25.0.Results:1.BaselineThere was no significant difference between the two groups in age,course of disease,distribution of severity of illness,24-hour urinary incontinence frequency before treatment,ICI-Q-SF score,I-QOL score and SDS score(P > 0.05),which was comparable.By stratification,the ICI-Q-SF scores of mild and moderate patients in the two groups before treatment were statistically significant(P > 0.05)and comparable.2.Total effective rateAfter two weeks of treatment,the total effective rate of the elongated needle group was94.29% and that of the filiform needle group was 74.28%.The difference between the two groups was statistically significant(P < 0.01).3.24 hours urinary incontinence timesAfter 2 weeks of treatment,the 24-hour urinary incontinence frequency of both groups obviously decreased compared with that before treatment(P < 0.01).Group comparison,after two weeks of treatment,the number of urinary incontinence in 24 hours in the elongated needle group was obviously lower than that in the filiform needle group(P < 0.05).4.ICI-Q-SF scoreICI-Q-SF scores of the two groups both decreased after 1 week and 2 weeks of treatment compared with those before treatment(P < 0.01).Comparison between groups,after one week of treatment,the ICI-Q-SF score of the elongated needle group was lower than that of the filiform needle group(P < 0.05);after two weeks of treatment,the ICI-Q-SF score of the elongated needle group was obviously lower than that of the filiform needle group(P < 0.01).5.I-QOL scoreI-QOL scores of the two groups both obviously increased after 1 week and 2 weeks of treatment compared with those before treatment(P < 0.01).Comparison between groups,the two groups’ I-QOL scores has no significant difference after one week(P > 0.05).However,I-QOL scores of the elongated needle group were higher than those of the filiform needle group after two weeks(P < 0.05).6.SDS scoreSDS scores of patients in both groups were obviously lower than those before treatment(P < 0.01).After 2 weeks of treatment,the SDS score of the elongated needle group was lower than that of the filiform needle group(P < 0.05).7.ICI-Q-SF scores of different severityComparing the ICI-Q-SF scores of mild and moderate patients with those before treatment,the scores of both groups were significantly reduced(P < 0.01).After the treatment,ICI-Q-SF scores of mild and moderate elongated needle group were significantly lower than those of filiform needle group(P < 0.01).8.Incidence of adverse reactionsThe incidence of adverse reactions was 8.57% in the elongated needle group and 5.71%in the filiform needle group.After continuous correction chi-square test,the two groups has no significant difference(P > 0.05).Conclusions:1.Two kinds of acupuncture treatments with different depths combined with Kegel training can improve the clinical symptoms of female stress urinary incontinence,and the curative effect of elongated needle acupuncture is better than that of filiform needle acupuncture.2.Both acupuncture with elongated needle and with filiform needle can reduce the number of urine leakage and relieve depression and anxiety,and the curative effect of elongated needle is better than that of filiform needle.3.Both acupuncture with elongated needle and with filiform needle can improve patients’ quality of life.After one week of treatment,the improvement of quality of life of elongated needle and filiform needle is equal,and after two weeks of treatment,the improvement of quality of life of elongated needle group is more remarkable,which indicates that with the increase of treatment time,the advantages of improvement of quality of life of elongated needle group will also expand.4.The curative effect of elongated needle acupuncture on patients with mild stress urinary incontinence and patients with moderate stress urinary incontinence is better than that of filiform needle acupuncture.5.The treatment of female stress urinary incontinence with elongated needle combined with Kegel training is effective,safe and reliable,with low adverse reaction rate and good patient compliance,which contributes a new idea and scheme for clinical treatment of this disease. |
