Study On The Effect Of Kegel Training Combined With TCM Directional Dialysis On Stress Urinary Incontinence In Middle-aged And Elderly Women

Objective: By observing the effects of Kegel training,TCM directional permeation and the combined intervention of the two on the improvement of symptoms and quality of life in middle-aged and elderly women with mild to moderate stress urinary incontinence,the effectiveness and safety of Kegel training combined with TCM directed permeation were discussed.It aims to find a more ideal non-surgical treatment for female patients with mild to moderate stress urinary incontinence,and at the same time provide new ideas and theoretical basis for clinical treatment.Methods: In this study,96 patients from a tertiary hospital in Changchun City were randomly grouped by random number table method,with 32 cases in each group.The three groups used Kegel training,TCM directional permeation therapy and a combination of the two to provide nursing intervention for 4 weeks.The lh urine pad test was performed before and after the intervention,and the effect of the nursing intervention was evaluated by the International Advisory Committee on Urinary Incontinence Questionnaire(ICI-Q-SF)and the Urinary Incontinence Quality of Life Scale(I-QoL),and finally the SPSS Statistics 21 software was used for statistical analysis.Results: 1.Baseline comparison: the three groups of subjects were in age(59.41±5.86,60.28± 5.30,60.28± 6.46);There were no significant differences in the distribution of body mass index(23.82± 3.36,23.44±3.46,24.09±3.14),number of deliveries [1(1,1),1(1,1),1(1,1.5)],mode of delivery,and course of disease(5.60±2.39,5.78±2.81,5.93±2.58)(P>0.05),and the 1-hour urine pad test(7.67± 1.44,7.45± 1.59,1.59,7.62±1.55);ICI-Q-SF score(12.56± 2.23,12.69± 2.19,12.63±2.00);The I-QoL scores(58.63±4.78,59.33± 4.80,58.98± 4.95)were not statistically significant(P>0.05)and were well comparable.2.Treatment results:(1)Results of 1h urine pad test: After the end of the nursing intervention,the results of urine leakage in the three groups of 1-hour urine pad tests were 5.98± 1.90,4.72± 1.64,3.49 ± 1.28,respectively.Intra-group comparison: Compared with before the start of nursing intervention,the 1h urine pad test leakage in the three groups was reduced to varying degrees,and the difference was statistically significant(P<0.05),and the intergroup comparison: the 1h urine pad test leakage in the combined intervention group was lower than that in the TCM directional permeation treatment group and the Kegel training group,and there was a significant difference between the three groups(P<0.05).(2)ICI Q-SF score: After the end of the nursing intervention,the scores of the three groups were 8.22± 3.09,6.53± 3.24,4.97± 2.98,respectively.Intra-group comparison:Compared with before the start of nursing intervention,the ICI-Q-SF scores of the three groups were significantly reduced to varying degrees(P<0.05),and the ICI-Q-SF scores in the combined intervention group were lower than those in the TCM directional permeation treatment group and lower in the Kegel training group,and there were significant differences between the three groups(P<0.05).(3)I-QoL score: After the end of the nursing intervention,the scores of the three groups were 63.88 ± 5.41,68.32 ± 5.41,72.08 ± 5.36,respectively.Intra-group comparison: Compared with before the start of nursing intervention,the I-QoL score of the three groups was reduced to different degrees,and the difference was statistically significant(P<0.05),and the I-QoL score in the combined intervention group was lower than that in the TCM directional permeation treatment group and lower in the Kegel training group,and there was a significant difference between the three groups(P<0.05).(4)Clinical efficacy evaluation:After the end of the nursing intervention,the total effective rate of Kegel training group was 65.63%(21/32),the total effective rate of TCM directional permeation treatment group was 84.38%(27/32),and the total effective rate of the combined intervention group was 93.75%(30/32),the effect of the combined intervention group was better than that of the TCM directional permeation treatment group was better than that of Kegel training group,and the clinical efficacy between the three groups was statistically different(P<0.05).3.Safety evaluation: All 96 subjects in this trial completed the nursing intervention of each group as scheduled,and there were no adverse reactions occurred during the study,and there were no cases of shedding or elimination.Conclusions: Kegel training,directional penetration therapy of traditional Chinese medicine and combined application of both have certain therapeutic effects on mild to moderate stress urinary incontinence in middle-aged and elderly women.However,compared with the single application of Kegel training or directional penetration therapy of traditional Chinese medicine,the combination of the two can alleviate the symptoms of urinary incontinence more significantly,improve the quality of life more,and the therapeutic effect is better.In this study,during the nursing intervention,the three groups of subjects did not appear adverse reactions,which can be used as a safe and effective method for clinical promotion.
