| Objectives: Through the analysis of clinical data of burn scar ulcer canceration,the clinical characteristics diagnosis and treatment experience of burn scar ulcer canceration were summarized,so as to improve the level of early prevention,early diagnosis and early treatment of burn scar ulcer canceration and the quality of life of these patients Methods: To establish criteria for diagnosis,inclusion and exclusion of burn,burn scar ulcer and burn scar ulcer canceration,and to formulate statistical tables of burn scar ulcer and burn scar cancer medical records.From January 1991 to January 2021,60 cases of burn scar ulcer and 60 cases of burn scar cancer hospitalized in the Plastic Surgery Department of the First Affiliated Hospital of Guangxi Medical University and the Burn plastic Surgery Department of the 924 Hospital of the Joint Logistic Support Force of the PEOPLE’s Liberation Army were collected.And the two groups of patient’s sex,age,occupation,lifestyle(smoking),basic diseases(diabetes),family history of cancer,burns(burn cause,site,depth,area,treatment),the types of scarring,burn scar ulcer related(ulcer site,depth,saturation level,treatment),burn scar cancer related(ulcer site,depth,saturation level,Clinical data including course of disease,pathological classification,lymph node metastasis,treatment plan and prognosis were analyzed.Chi-square test was used for comparison of constituent ratio.SPSS 22.0 software was used for statistical processing.Results: 1.General statistical findings:(1)Among 60 patients with burn scar ulcer,(1)the male to female ratio was 2.3:1,and the age at diagnosis was 4-70(39.45±18.23)years old.The course of ulcer disease is 0.25 to 55(12.98±14.25)years.The area of ulcer was 0.25-150(24.38±36.95)cm2.(2)Lifestyle: Smoking rate 8.33%.(3)Burn causes: burn and scald accounted for 78.33%.(4)Ulcer site: lower limbs,88.32%.(5)Enlargement of the regional lymph nodes: 8.33%.(6)Treatment: 98.33% were treated with skin flap or skin flap.Amputation rate was 1.67%.(2)Among 60 patients with burn scar ulcer cancer,(1)Male to female ratio was 5:1,age at diagnosis was 24-79(52.55±12.18)years old.The course of ulcer disease is 1-56(32.20±14.05)years.The ulcer area was 5.4-375.0(82.65±120.13)cm2.(2)Lifestyle: Smoking accounts for 25%.(3)Causes of burns: mainly burns and scalds,accounting for 96.7%(4)Ulcer site: frequent occurrence of lower limbs,accounting for 85.00%.(5)Enlargement of the regional lymph nodes:33.33%.(6)Treatment: 78.33% were treated with skin flap or skin flap.Amputation rate was 21.67%,and recurrence rate was 1.67%.2.Chi-square test was used for statistical analysis of the related factors affecting the carcinogenesis of burn scar ulcer.The results showed that age,lifestyle(smoking),burn area and incubation period were P <0.05,the difference was statistically significant,and were the related factors affecting the carcinogenesis of burn scar ulcer.Gender and burn site had no statistical significance on burn scar ulcer canceration.Conclusions: 1.Middle-aged and elderly patients have a greater risk of burn scar ulcer cancer than younger patients.2.Smoking can increase the risk of canceration of burn scar ulcers.3.The larger the area and the longer the course of burn scar ulcers,the greater the possibility of canceration of burn scar ulcers. |
