Correlation Between Coronary Stenosis And FPR,MPVLR,ApoB/ApoAI In Patients With Coronary Heart Disease Complicated With Subclinical Hypothyroidism

Objective: By analyzing and comparing the ratio of Fibrinogen to Prealbumin,the ratio of Mean platelet volume to Lymphocyte,the ratio of Apolipoprotein B to Apolipoprotein AI and various blood biochemical parameters in patients with coronary atherosclerotic heart disease complicated with subclinical hypothyroidism,the correlation between the degree of coronary artery stenosis in patients with coronary heart disease complicated with subclinical hypothyroidism was discussed by Gensini score,To provide a theoretical basis for early clinical intervention.Methods : A total of 314 patients hospitalized in the first ward of cardiovascular medicine of the Affiliated Hospital of Guilin Medical College from June 2018 to March 2021 were collected retrospectively.According to the results of coronary angiography and thyroid function test,they were divided into coronary heart disease with subclinical hypothyroidism group(group A,n=113,subclinical hypothyroidism was first diagnosed and did not take drug treatment),coronary heart disease with normal thyroid function group(group B,n=103)and coronary angiography and thyroid function were normal group(control group,group C,n=98);Gensini score was used to evaluate the degree of coronary artery stenosis;Pearson correlation analysis was used to analyze the correlation between FPR,MPVLR,Apo B/Apo AI and Gensini score;Multivariate logistic regression was used to analyze the main risk factors of coronary heart disease,and Medcalc software was used to draw the receiver operating characteristic curve(ROC curve)to evaluate the diagnostic value.Results: Through the comparative analysis of baseline data,there were significant differences among the three groups in gender,age,previous history of hypertension,MPVLR,Apo B/Apo AI,Lymph,Apo AI,WBC,RBC,HGB,NEUT,HCT,FT4,TSH,TG,CHOL,LDL-C and Gensini scores(P<0.05);Gensini score was positively correlated with FPR and Apo B/Apo AI in patients with coronary heart disease complicated with subclinical hypothyroidism(r=0.292,P=0.003;r=0.224,P=0.017);Multivariate logistic regression analysis showed that MPVLR,Apo B/Apo AI,MPV,HCT and LDL-C were the main risk factors of coronary heart disease complicated with subclinical hypothyroidism.The area under ROC curve(AUC)of multivariate logistic regression model was0.772(95%CI 0.710-0.826,P<0.0001),Youden index was 42.96%,sensitivity was 74.34% and specificity was 68.63% respectively.Conclusions:(1)There is a correlation between coronary heart disease and subclinical hypothyroidism,which may increase the degree of coronary stenosis in patients with coronary heart disease;(2)Subclinical hypothyroidism can adversely affect lipid metabolism in patients with coronary heart disease,thereby increasing the risk of cardiovascular disease;(3)Subclinical hypothyroidism can affect the parameters of cells in patients’ blood;(4)FPR,Apo B/Apo AI are positively correlated with Gensini score in patients with coronary heart disease complicated with subclinical hypothyroidism,which has a certain clinical significance in evaluating the severity of coronary artery disease;(5)Multivariate logistic regression analysis showed that MPVLR,Apo B/Apo AI,MPV,HCT and LDL-C were the main risk factors of coronary heart disease complicated with subclinical hypothyroidism,and have certain predictive value for coronary heart disease complicated with subclinical hypothyroidism.
