| Objective1.Analyze the influencing factors of quality of life in patients with ischemic stroke, and explore the mediating role of hope level in the influence of social support and family function on quality of life in patients with ischemic stroke.2.To verify the effect of nursing intervention based on Snyder hope theory on the quality of life of patients with ischemic stroke.Methods1.From March to October 2021,241 patients with ischemic stroke were selected from the Department of Neurology of a third-class hospital in Inner Mongolia.General condition questionnaire,Social support rating scale,Family intimacy and adaptability scale,Herth hope scale and Quality of life rating scale were used to investigate.2.From June to October,2021,68 patients with ischemic stroke in the Department of Neurology of a third-class hospital in Inner Mongolia Autonomous region were randomly divided into two groups: the experimental group(n = 34)and the control group(n = 34).According to the investigation and analysis, the level of hope plays an intermediary role in social support and family function on the quality of life, indicating that the level of hope plays an important role in improving the quality of life of patients with ischemic stroke, and Snyder hope theory is applied and studied with the goal of producing a whole promising cognition. Therefore, based on Snyder’s hope theory and referring to the factors affecting the quality of life of patients with ischemic stroke,Construct a nursing intervention plan to improve the quality of life of patients. The control group received routine nursing intervention for ischemic stroke and the experimental group received nursing intervention based on Snyder hope theory.Eight nursing interventions were conducted in the study. The patients were evaluated before and after the intervention by using Social support rating scale,Family intimacy and adaptability scale,Herth hope Scale and Quality of life rating scale, and the intervention effect was discussed and analyzed.Results1.The overall average of hope level of patients with ischemic stroke was(30.72 ±4.18),indicating that the hope level of patients was in the middle level,in which patients had the lowest score of positive attitude towards reality and future, and the highest score of maintaining close relationship with others.social support and family function were at a low level, with overall averages of(34.49 ±5.58)and(92.62 ±14.49),respectively.The overall average score of quality of life was(371.26 ±103.40),and the average scores of each dimension from low to high were RP,RE,GH,PF,VT,MH,SF and BP.Age is related to quality of life(r=-0.358,P<0.05).Gender,marital status, living style, family relationship, number of children,self-care and understanding of diseases had an impact on the quality of life(P < 0.05).Social support was positively correlated with hope level and quality of life(r= 0.537,P < 0.01).Family function was positively correlated with hope level and quality of life(ritual 0.494 P < 0.01).Hope level played an intermediary role between social support and family function on quality of life, and indirect effects accounted for 93.53% and 60.93% of the total effects, respectively.Bootstrap test showed that the 95% confidence interval of mediating effect was(2.21,6.16)and(0.72,2.02)respectively.2.Before intervention, there was no significant difference in general data, social support, family function, hope level and quality of life between the two groups of patients with ischemic stroke(P > 0.05).After intervention, there were significant differences in the scores of social support, family function and hope level between the experimental group and the control group of patients with ischemic stroke(P < 0.05),and the score in the experimental group was higher than that in the control group(P < 0.05).After intervention, there were significant differences in the dimensions of VT,SF,MH and SF-36 between the two groups, and the score of the experimental group was better than that of the control group.Conclusion1.In patients with ischemic stroke, the mediating role of hope level in social support and family function on quality of life suggests that clinical nurses can focus on improving hope level when improving the quality of life of patients with ischemic stroke, formulate corresponding nursing plans and intervene as soon as possible,so as to mobilize the hope value of patients and improve the enthusiasm of patients,So as to improve the quality of life of patients. In patients with ischemic stroke, the level of hope plays an intermediary role in social support for quality of life and family function on quality of life, respectively. It is suggested that when nursing patients with ischemic stroke, clinical nurses should focus on evaluating the patients’ hope level, actively take effective plans and intervene as soon as possible,in order to mobilize the patients’ expectation value and improve the enthusiasm of the patients.and then improve the quality of life of the patients.2.Based on Snyder’s hope theory and combined with the influencing factors of quality of life of patients with ischemic stroke, the nursing intervention scheme has a significant effect on patients. After the intervention, the patients’ hope level is improved and the quality of life is also improved. |
