| Objective To provide reference for the intervention of frailty in elderly patients with type 2 diabetes mellitus(T2DM),we preliminarily analyzed the frailty situation and the influencing factors of frailty.Methods A total of 254 T2 DM patients hospitalized in department of Gerontology and Department of Endocrinology,Affiliated Hospital of Qing hai University(November 20,2020-December 20)were selected according to inclusion and exclusion criteria.General data,laboratory examination,bone mineral density(BMD)and carotid intima thickness of elderly T2 DM patients were collected.Fried criteria were used to assess frailty in all elderly T2 DM patients and the results were recorded.According to the frailty score,the subjects were divided into 3 groups: group A: T2 DM group without frailty(meeting 0 Fried criteria);Group B: pre-frailty T2 DM group(1-2 Fried criteria);Group C: T2 DM frailty group(≥3 Fried criteria).The general data and clinical indicators of T2 DM patients were statistically analyzed,and the influencing factors of frailty were calculated.The software SPSS25.0 was used for statistical description.Results(1)In 254 elderly T2 DM patients,the detection rate of pre-frailty was41.7%,and the detection rate of frailty was 19.7%.(2)There were significant differences in basic data and disease status among group A,B and C: BMI,age,sex,drinking history,education level,insulin use,multiple medication and comorbidity(P< 0.05).Group C and group A were further compared,and group C and group B were compared.Group C was older than group A and B.The BMI of group C was lower than that of groups A and B.(3)There were statistically significant differences in LDL-C,Cys C,ESR,Hb,D-D,CPR,ALB and osteoporosis among group A,B and C in laboratory examination and imaging data(P < 0.05).Further comparison was made between group C and group A and between group C and group B: there were statistical differences in LDL-C,Cys C,ESR,Hb,D-D and ALB between group C and Group A(P < 0.05),and there were statistical differences between group C and Group B in D-D(P < 0.05).(4)Further analysis of the relationship between BMD values in different parts of group A,B and C: BMD values in different parts of group C were lower than those in group B and group A(P < 0.05).The BMD value of 1/3 of femoral neck and forearm in group B was lower than that in group A(P < 0.05).(5)Multifactor regression analysis: age,osteoporosis,decreased BMI,decreased ALB,accelerated ESR,increased CPR expression,increased Cys C level,use of insulin,comorbidity had statistical significance in the occurrence of frailty(P < 0.05).Conclusion 1.The detection rate of pre-frailty in elderly T2 DM patients was41.7%,and the detection rate of frailty was 19.7%.2.Age,osteoporosis,decreased BMI,decreased ALB,accelerated ESR,increased CPR expression,increased Cys C,use of insulin,and comorbidity are the risk factors for frailty in elderly T2 DM patients. |
