Purpose:To explore the application value of multimodal image three-dimensional fusion technology in the operation of intracranial central gyrus meningioma.This paper introduces our experience in preoperative planning and surgical operation for central gyrus meningioma,and describes the significance of intraoperative protection of the returning veins and important tissues,in order to completely remove the tumor and protect the brain function to the greatest extent.Method:From April 2019 to may 2021,88 patients with meningiomas in the central region were included in the study,including 46 in the study group and 42 in the control group.Patients in the control group underwent routine imaging examination before operation,and neuronavigation assisted tumor localization and surgical resection during operation.The research group used 3D slicer software to fuse and reconstruct the image data to construct three-dimensional models of tumor,blood vessel,cortex and skull.According to the 3D fusion results,the anatomical relationship between the tumor and the surrounding blood vessels and functional areas was observed.So as to design the best surgical approach to assist in tumor resection and functional area protection.Before the operation,Sina software was used to design the scalp incision assisted by body surface projection.Combined with the actual tumor site and its adjacent structures during the operation,the effect of preoperative reconstruction was verified.The accuracy of tumor localization,degree of tumor resection and limb function after operation were analyzed and compared between the two groups.Result:1.The research group used Sina software to assist in preoperative incision design and intraoperative craniotomy for verification.Of the 46 cases,45 cases(97.83%)had accurate localization and 1 case(2.17%)had inaccurate localization;The control group was located by neuronavigation.In the control group,neuronavigation was routinely used for localization.Among 42 cases,41 cases(97.62%)were accurate in localization and 1 case(2.38%)was inaccurate.There was no significant difference between the two groups in localization accuracy(P>0.05).2.In terms of tumor resection,46 cases in the study group,40 cases(86.96%)completed Simpson grade Ⅰ resection and 6 cases(13.04%)completed Simpson grade Ⅱ resection.There were 42 cases in the control group,of which 35 cases(83.33%)completed Simpson Grade Ⅰresection and 7 cases(16.67%)completed Simpson Grade Ⅱ resection,there was no significant difference between the study group and the control group(P>0.05).3.The limb function of the patients was evaluated with the limb muscle strength scale 24 hours after operation.Of the 46 patients in the study group,40 patients(86.96%)had the same or improved limb muscle strength after operation,of which 32 patients(69.57%)had normal limb function before operation and no limb dysfunction after operation;Limb function improved significantly in 8 cases(17.39%);Muscle strength decreased in 6 cases(13.04%).Control group:Among 42 patients,28 patients(66.67%)had the same or improved limb muscle strength after operation as before,of which 24 patients(57.14%)had the same muscle strength after operation as before,4 patients(9.52%)had improved limb muscle strength,and 14 patients(33.33%)had decreased limb muscle strength after operation.In terms of the same and improved limb muscle strength after operation and before operation,the study group was superior to the control group,and the difference between the study group and the control group was statistically significant(P<0.05).Conclusion:The three-dimensional reconstruction technology of multimodal image fusion can intuitively,multi dimensionally,dynamically and stereoscopically display the tumor and its adjacent anatomical structures,accurately locate the tumor and functional area,and assist in the design of the surgical approach,so that the tumor can be completely removed and the surrounding brain tissue and blood vessels can also be well protected.At the same time,it can minimize the postoperative neurological dysfunction and ensure the patients have a high quality of life.Multimodal image coincidence reconstruction technique has high application value in the operation of central meningioma. |
