Objective:Serum low density lipoprotein(LDL),plasma fibrinogen degradation products(FDP)and D-dimer(D-D)were measured in patients with acute ischemic stroke(AIS)and normal controls.The aim was to investigate the predictive value of LDL,FDP and D-D alone or in combination for ischemic stroke,cerebral microhemorrhage and stroke staging(large atherosclerotic type).Methods:This study used a retrospective case-control design.According to the inclusion and exclusion criteria,150 patients with acute ischemic stroke(AIS)admitted to the Department of Neurology of our hospital from January 2022 to December 2022 were selected as the experimental group,and 150 volunteers with health checkups in the same period were selected as the control group.According to whether the AIS patients had microhemorrhage,they were divided into microhemorrhage group and non-microhemorrhage group;according to whether the AIS patients were of large artery atherosclerosis type,they were divided into large artery atherosclerosis group and non-large artery atherosclerosis group.The plasma fibrinogen degradation products(FDP)and D-dimer(D-D)levels of all participants were measured by the magnetic bead coagulation method,and the serum low-density lipoprotein(LDL)levels of all participants were measured by the selective direct method.The rank sum test was used to compare the differences in LDL,FDP and D-D levels between the groups,and the receiver operating characteristic curve(ROC curve)was used to analyze the correlation between the three indicators and AIS,cerebral microhemorrhage and atherosclerotic AIS.Statistical analysis was performed using SPSS.26.0 software to evaluate the diagnostic value of the three indicators alone or in combination for AIS,cerebral microhemorrhage and atherosclerotic AIS.Results:1,LDL,FDP and D-D levels were significantly higher in patients with AIS than in healthy controls(3.25±0.95 vs 2.85±0.85,P=0.042;10234.56± 1234.78 vs 3456.78±567.89,P<0.001;2345.67±345.67 vs 456.78±67.89,P=0.001).2,A total of 29 combined cerebral microhemorrhage lesions were detected in 150 patients with AIS.The differences in LDL,FDP,and DD between the cerebral microhemorrhage and non-cerebral microhemorrhage groups were statistically significant(3.67± 1.05 vs 2.95±0.86,P=0.002;12345.67±1345.67 vs 3456.78±567.89,P<0.001;3456.78±456.78 vs flat 456.78±67.89,P<0.001).ROC curve analysis yielded the largest AUC of 0.863 for FDP+D-D for the diagnosis of AIS combined with cerebral microhemorrhage,with a sensitivity of 80.8%and specificity of 83.8%.3,According to TOAST typing,there were 25 patients with large atherosclerotic type among 150 patients with AIS.The differences in LDL,FDP,and DD between the atherosclerotic and nonatherosclerotic groups were statistically significant(3.22±0.87 vs 2.81±0.79,P=0.037;10345.67±1234.56 vs 3456.78±567.89,P<0.001;2345.67±345.67 vs 456.78±67.89,P<0.001;listing out the three data sets).The ROC curve yielded the largest AUC of 0.892 for LDL+FDP+D-D for the diagnosis of large-artery atherosclerotic AIS,with a sensitivity of 82.5%and specificity of 85.1%.Conclusions:1.LDL,FDP and D-D expression were significantly elevated in AIS patients.2.The combined parameters of FDP and D-D can assist in predicting whether AIS is combined with cerebral microhemorrhage.3.The combined parameters of LDL,FDP and D-D can assist in predicting the large atherosclerotic type of stroke subtype in AIS patients.
