Study On The Clinical Experience Of Professor Liu Guirong In Treating Palpitation With Anxiety And Depression

Objective:In this study,we collected and screened the medical records of professor Liu guirong’s palpitation patients with depression syndrome,and used various modern data mining techniques for statistical analysis,to sum up professor Liu guirong’s clinical experience in treating palpitation with depression.By studying the experience of famous Chinese medicine and inheriting the thought of famous Chinese medicine,it can provide reference for the cognition and treatment of palpitation with depression syndrome.Methods:This study collected the cases of palpitation with depression syndrome treated by Professor Liu guirong from December 2018 to December 2022 in the specialist clinic of the Department of Physical and mental medicine of the Affiliated Hospital of Shandong University of traditional Chinese medicine(Shandong Provincial Hospital of traditional Chinese medicine)and the specialist clinic of Shandong New Zhonglu Hospital(the former Zhonglu Hospital of Shandong University of traditional Chinese medicine),medical records were selected according to inclusion criteria and exclusion criteria,and excluded according to exclusion criteria.The general data of these patients(including name,age,sex,etc.),clinical symptoms(all patients had palpitation),tongue picture(divided tongue coating and tongue color),pulse condition,prescription of Chinese traditional medicine,etc.were collected and recorded into WPS Excel by two graduate students,to establish the case database of palpitation with depression syndrome.A total of 103 cases were included in the medical records,and then the data were imported into the ancient and modern medical records cloud platform system,and after the standardized pre-processing of the information such as prescriptions and drugs that were imported into the platform data,the clinical symptoms,tongue conditions,pulse conditions,frequency of prescriptions and medicines,the attributes of medicines(including meridian tropism,flavor,efficacy,etc.)and the analysis of drug association were analyzed and mined,the clinical experience of treating palpitation with depression syndrome was analyzed according to the oral teaching and teaching materials of professor Liu guirong.Results:1.General information: The statistics showed that the female patients with palpitation accompanied by depression were more than the male patients.The age of onset of palpitation was 30 ～ 39 years old and 50 ～ 59 years old,the proportion was more than20%,to the two age groups as the center,to both ends of the declining trend.2.Four diagnostic information: There were 31 symptoms with frequency ≥5,except palpitation,the 5 most frequent were poor sleep,short temper,sweating,frightening and nervous.There were 10 kinds of tongue texture involved,among which 5 kinds of tongue texture with frequency ≥5 were dark,dark red,dark green,dark stasis and dim.There are17 kinds of tongue coating involved,among which 7 kinds of tongue quality with frequency ≥5 are white,thick,thin,yellow,dry,thick and greasy.There are 15 kinds of pulse,9 kinds of pulse frequency ≥5,from more to less,they are pulse astringency,pulse thinness,pulse urgency,pulse string,pulse sink,pulse tight,pulse slippery,pulse weak,pulse hard.3.Diagnosis: 103 cases were included in this study,of which 103 cases were all in the state of anxiety,accounting for 100%,of which 45 cases were also in the state of depression,accounting for 43.7%.4.Application of traditional Chinese medicine: Through the statistical analysis of the prescription of the cases of palpitation with depression,the results were obtained,involving a total of 160 Chinese medicine.Among them,there are 29 kinds of traditional Chinese medicine with frequency ≥20,from most to least,they are Bupleurum chinense,Ginseng,Keel,Oyster,Ginger Pinellia Ternate,Scutellaria,Cassia Twig,Tuckahoe,Poria,Magnet,Liquorice,Morinda officinalis,Dragon’s teeth,Cinnabar,Astragalus,Baked liquorice,Polygala tenuifolia,Acorus gramineus,Tangerine Peel,Dannanxing,Schisandra,Fructus aurantii,Jujube,Rhubarb,Ginger,Gardenia,aconite,Lapis lazuli,Coptis chinensis.Through the analysis of the nature,taste and meridian tropism of these Chinese medicines,it was found that when professor Liu treated palpitation with depression syndrome,he used the drugs of warm and flat nature most(the frequency was more than 400),followed by the drugs of mild cold,cold nature and mild temperature,cold,heat,heat and cold drugs were used less-LRB-frequency less than 200),sweet drugs were used most,followed by pungent and bitter drugs(frequency more than 600),and other drugs were used less;Most of the drugs were used in heart meridian and lung meridian,followed by spleen meridian,liver meridian,kidney meridian and stomach meridian(the frequency was more than 500),and less in gallbladder meridian,large intestine meridian and bladder meridian The drug effects of the first 10 frequencies were in order from most to least: tranquilizing and tranquilizing,calming the liver and suppressing yang,promoting body fluid and nourishing blood,tranquilizing and benefiting the mind,soothing the liver and relieving depression,calming the heart,elevating Yang-qi,clearing away heat and dryness,relieving exterior fever,purging fire and detoxifying.The first 10 kinds of Chinese traditional medicine in order of frequency are as follows: tonic drugs are the most,next is the solution surface class,the clearing heat class,the tranquilization class,the li-qi class,the water and dampness class,the calming liver and the relieving wind class,the resolving phlegm and relieving cough and asthma class,the activating blood and removing blood stasis class and the warming in class.The commonly used drug combinations were(Pinellia Ternate,Keel,Oyster,Bupleurum root,Scutellaria baicalensis Georgi)(Longtooth,Polygala tenuifolia,Acorus tatarinowii)(Magnet,Licorice)(Baked licorice root,Tangerine peel,Dananxing)(Poncirus trifoliate,Rhubarb,Gardenia,Lapis chloriti,schisandra chinensis,Salvia miltiorrhiza,Coptis chinensis,Dry ginger,Aconite).Conclusion:Professor Liu believes that the pathogenesis of palpitation accompanied by depression syndrome is the stagnation of triple energizer,the disorder of Qi and mental function,which is closely related to heart,lung,spleen and liver,the prescription is mainly composed of Bupleurum plus longgu muli decoction and a large number of mineral drugs to calm the nerves.
