Xuan Yun Ning Prescription In The Treatment Of Persistentpostural Perceptual Dizzinessin Terms Of Gallbladder Fire Depression And Spleen Deficiency And Phlegm Turbidity

Objective: To observe the clinical efficacy of Xuan Yun Ning Prescription in the treatment of persistent postural-perceptual dizziness(PPPD)caused by bile fire depression and phlegm turbidity,evaluate its effectiveness and safety,and explore its mechanism of action,so as to provide new syndrome differentiation and treatment methods for PPPD in traditional Chinese medicine.Methods: 70 patients with PPPD meeting the inclusion criteria were divided into treatment group and control group with 35 patients in each group by randomized control method.The treatment group was given paroxetine tablets combined with Xuan Yun Ning,and the control group was given paroxetine tablets.The treatment course of both groups was 4 weeks.Statistical software SPSS 27.0 was used to analyze and process the data.The scores of TCM syndrome score scale,Vertigo Disability Rating Scale(DHI),Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were calculated before and after treatment,and the therapeutic efficacy and safety of the two groups were compared.Results: There were 5 cases of shedding during the treatment.Finally,33 cases in the treatment group and 32 cases in the control group were included in the statistics.There was no significant difference in age,gender,course of disease and other general information between the two groups as well as the TCM syndrome score,DHI score,HAMA score and HAMD score before treatment(P < 0.05),indicating comparability.Clinical observation results: 1.Intra-group comparison of observation indicators:compared with before treatment,TCM syndrome score,DHI score,HAMA score and HAMD score were significantly decreased in the two groups,with statistical significance(P < 0.05).2.Comparison between observation indicators: After 28 days of treatment,the total score of TCM syndrome,DHI score,HAMA score and HAMD score of the treatment group were lower than those of the control group(P < 0.05).In terms of the individual TCM symptoms,the scores of the treatment group were lower than those of the control group(P < 0.05).There were no significant differences in dizziness,nausea,phlegm and salivation,poor sleep and loose stool between the treatment group and the control group(P > 0.05).3.Comparison of curative effect: The effective rate of the treatment group was better than that of the control group in the improvement of TCM syndrome efficacy,dizziness degree,anxiety symptoms and depression symptoms(P < 0.05).Conclusion: Xuan Yun Ning can improve the degree of dizziness,frequency and duration of attack in PPPD patients with bile fire depression and phlegm-turbidity,effectively improve TCM syndromes,relieve anxiety and depression,less adverse reactions,good safety,and definite clinical efficacy.Treating PPPD from bile fire depression,spleen deficiency and phlegm-turbidity is an effective clinical treatment idea.
