| Objective:Based on the Opening,Closing and Pivoting of Three Yin and Three Yang in traditional Chinese medicine,this study aims to explore the clinical efficacy and application value of using Xuefu Zhuyu decoction combined with Suanzaoren decoction to treat elderly patients with chronic insomnia of the Shaoyin and Shaoyang types.A comparative analysis of the therapeutic effects was conducted against the control group receiving estazolam,in order to investigate the clinical efficacy and application value of the theory of Opening,Closing and Pivoting of Three Yin and Three Yang in the treatment of chronic insomnia in the elderly population.Method:A total of 82 elderly patients with chronic insomnia who met the inclusion criteria were randomly divided into the experimental group(n=41)and the control group(n=41).The experimental group received treatment with Xuefu Zhuyu decoction combined with Suanzaoren decoction,while the control group received estazolam tablets.The treatment period lasted for 4 weeks.Before and after the treatment,the Pittsburgh Sleep Quality Index(PSQI),Insomnia Severity Index(ISI),and Traditional Chinese Medicine(TCM)Syndrome Scale for the Shaoyin and Shaoyang types of insomnia were completed.Safety indicators were also measured.Data were collected and organized in Excel,and statistical analysis was performed using R software(version 4.1.1).The clinical efficacy of the two groups was compared and evaluated based on the analysis results.Any adverse reactions that occurred during the trial were promptly recorded and evaluated for safety in both groups.Results:A total of 38 patients in the experimental group and 39 patients in the control group completed the observation,with 3 and 2 dropouts,respectively.Comparative analysis of the data between the two groups revealed the following results:1.There were no significant differences in age,gender,and course of illness between the experimental and control groups(P>0.05).The baseline scores of the PSQI,ISI,and TCM Syndrome Scale for the Shaoyin and Shaoyang types of insomnia were also similar between the two groups(P>0.05),indicating comparability.2.After 4 weeks of treatment,significant differences were observed in the total scores of the PSQI,ISI,and TCM Syndrome Scale for the Shaoyin and Shaoyang types of insomnia between the two groups(P<0.05),with lower scores in the experimental group.3.In terms of clinical efficacy,the total apparent effectiveness rates and total effective rates were 39.47%and 86.84%,respectively,in the experimental group,and 20.51%and 71.79%,respectively,in the control group,showing a significant difference between the two groups(P<0.05).4.In terms of TCM syndrome efficacy,the total apparent effectiveness rates and total effective rates were 31.58%and 92.11%,respectively,in the experimental group,and 7.69%and 69.23%,respectively,in the control group,indicating a significant difference(P<0.05).5.Adverse reactions occurred in both groups during the trial,with an incidence rate of 5.26%in the experimental group and 15.38%in the control group,demonstrating a lower incidence rate of adverse reactions in the experimental group.Conclusion:1.Based on the theory of Opening,Closing and Pivoting of Three Yin and Three Yang,this study selected elderly patients with chronic insomnia of the Shaoyin and Shaoyang types and treated them with Xuefu Zhuyu decoction combined with Suanzaoren decoction and estazolam,respectively.The results showed that the combined treatment with Xuefu Zhuyu decoction and Suanzaoren decoction exhibited better overall clinical efficacy in improving sleep quality,sleep efficiency,reducing sleep disorders and sleep onset difficulties,and improving daytime functioning in patients with chronic insomnia.2.Compared with estazolam,the combined treatment with Xuefu Zhuyu decoction and Suanzaoren decoction showed better overall efficacy in TCM syndrome improvement,especially in relieving difficulties in falling asleep,difficulty staying asleep,dry throat,vivid dreams,chest and hypochondriac fullness,palpitations,bitter taste in the mouth,dizziness,and night sweats,with a lower incidence of adverse reactions and higher safety.3.This study achieved expected results in the treatment of chronic insomnia of the Shaoyin and Shaoyang types in the elderly population guided by the theory of Opening,Closing and Pivoting of Three Yin and Three Yang,demonstrating the application value and practical significance of the theory in clinical practice. |
