Objective:To observe the clinical effect of Tongbianshengji Decoction on preventing and treating defecation difficulty after operation of damp-heat mixed hemorrhoids.Methods:Seventy patients admitted to the Department of Traditional Chinese Medicine Affiliated to Shandong Provincial Hospital Affiliated to Shandong First Medical University from December 2021 to October 2022 for external stripping and internal ligation to treat mixed hemorrhoids(damp-heat type)were selected and randomly divided into observation group and control group according to the order of admission,with 35 cases in each group.The control group was given conventional treatment and nursing measures,and the observation group was given oral Tongbianshengji Decoction on the first day after the operation on the basis of conventional treatment,one dose a day,one hour in the morning and evening after meals,respectively,for continuous intervention for 7 days.Symptom scores of the two groups at 3,5 and 7 days after surgery were recorded by using the symptom scale of difficult defecation,including the main symptoms(defecation effort,defecation time,stool characteristics,anal feeling of falling and lack of defecation)and minor symptoms(anal pain,anal bleeding after defecation,abdominal distension).The occurrence of difficult defecation and enema in the 1 week after surgery were recorded in the two groups.The wound healing time of the two groups was recorded.Adverse reactions during the intervention were recorded completely.SPSS26.0 was used for statistical analysis of all data.Results:1.There were no significant differences in gender,age,internal hemorrhoids score,number of external hemorrhoids and preoperative defecation habits between the two groups(P>0.05),indicating comparability.2.The incidence of postoperative difficulty in defecation was 8.82%in the observation group and 29.41%in the control group,and the difference was statistically significant(P<0.05).3.Within 1 week after operation,the incidence of enema was 2.94%in the observation group and 17.65%in the control group,the difference was statistically significant(P<0.05).4.The time between the first defecation and the end of operation was statistically significant between the two groups(P<0.05).5.3 days after the operation,the defecation effort,defecation time and stool characteristics of the observation group were better than those of the control group(P<0.05),but there was no significant difference between the two groups in the sense of anal sagging and the sense of inadequate defecation(P>0.05);5 days after the operation,the observation group was better than the control group in defecation effort,defecation time,stool characteristics,anal sagging sensation and sense of incomplete defecation(P<0.05).7 days after the operation,the observation group was better than the control group in defecation effort,defecation time,stool characteristics,anal sagging sensation and sense of incomplete defecation(P<0.05).6.There was no significant difference in anal pain between the two groups at 3 and 5 days after surgery(P>0.05),and the pain degree of the observation group was less than that of the control group at 7 days after surgery(P<0.05).There was no significant difference in postoperative anal bleeding at 3 days between the two groups,but there were differences in postoperative bleeding at 5 and 7 days(P<0.05).The postoperative bleeding score of the observation group was lower than that of the control group.The abdominal distension score of the observation group was lower than that of the control group 3 days after surgery(P<0.05),and the abdominal distension degree of the two groups was not significantly different 5 and 7 days after surgery(P>0.05).7.The postoperative wound healing time of the two groups was different(P<0.05),and the healing time of the observation group was shorter than that of the control group.8.No adverse reactions occurred in both groups during treatment.Conclusion:Tongbianshengji decoction for patients with damp-heat mixed hemorrhoids after surgery can effectively reduce the incidence of difficulty defecation,reduce the rate of enema,reduce defecation effort,shorten defecation time,improve stool characteristics,alleviate postoperative anal sagging and defecation feeling,accelerate wound healing,in addition to reduce postoperative pain,reduce anal bleeding after defecation,It is also effective in relieving abdominal distension,and there is no adverse reaction during treatment.It is worth promoting and applying in clinical practice.
