| Objective This paper aims to analyze the etiology, pathogenesis, and medication rules of seborrheic alopecia through a combination of theoretical research and data mining, and to construct a clinical medication plan for the treatment of seborrheic alopecia in traditional Chinese medicine, providing theoretical and clinical guidance for the treatment of this disease. Methods Theoretical research includes a systematic review of the correlation between traditional Chinese medicine and the human body physiology, etiology and pathogenesis, syndrome differentiation and treatment principles,as well as Western medicine’s etiology, pathogenesis, and treatment of seborrheic alopecia. In terms of data mining, relevant literature and medical cases related to seborrheic alopecia were collected from databases such as CNKI,Wanfang, and VIP,and a medical case database was established and standardized. The frequency statistics, association rule analysis, cluster analysis, and complex network analysis functions in the Ancient and Modern Medical Case Cloud Platform were used to mine and analyze the medication of various syndrome types in the database.Results(1)Theoretical research: In terms of traditional Chinese medicine, this disease is related to the human body’s essence,qi,blood, and five organs in terms of its correlation with human physiology. In terms of etiology,it is related to congenital endowment, diet, emotions, etc. In terms of pathogenesis,it is related to the disease of the five organs, dampness and heat, blood, phlegm, etc. The syndrome differentiation mainly includes spleen and stomach damp-heat syndrome, liver and kidney deficiency syndrome, blood heat and wind dryness syndrome,as well as other syndromes such as blood deficiency causing wind syndrome. The treatment principles mainly focus on clearing heat and promoting diuresis, nourishing the liver and kidneys, and cooling the blood and dispelling wind.In addition, all five organs can be treated. In terms of Western medicine,in terms of etiology,it is related to factors such as genetics and abnormal hormone metabolism. In terms of pathogenesis,it is related to signaling pathways such as androgen, type II5α-reductase,blood flow,Wnt/β-catenin,etc.In terms of treatment,it mainly involves the use of medications such as minoxidil and finasteride.(2)Data mining research: 3775 literature were retrieved, and finally 179 medical cases were included. The distribution of syndrome types of seborrheic alopecia was mainly as follows: spleen-stomach damp-heat syndrome(76 cases, accounting for 42.46%),liver-kidney deficiency syndrome(54 cases, accounting for 30.17%),blood-heat wind-dryness syndrome(21 cases, accounting for 11.73%),and other syndrome types(28 cases, accounting for15.64%).(1)Spleen-stomach damp-heat syndrome: the top three frequently used Chinese medicines were poria cocos(39 times,51.32%),coix seed(35 times,46.05%),and licorice(34 times,44.74%).The top macrocephala, cypress leaf, salvia miltiorrhiza, atractylodes lancea, angelica sinensis, and chuanxiong. The complex network analysis obtained a core prescription: poria cocos, coix seed, atractylodes macrocephala, licorice, and alisma orientalis.(2)Liver and kidney deficiency syndrome: The top three frequently used Chinese herbs are Lycium fruit(36 times,66.67%),Eclipta(33 times,61.11%),and Rehmannia root(25 times,46.30%).The top three frequently used Chinese herb functions are nourishing liver and kidney, cooling blood and stopping bleeding, and clearing heat and cooling blood. The four properties of frequently used Chinese herbs are neutral, cold, and warm. The five tastes of frequently used Chinese herbs are sweet, bitter, and pungent. The meridians of frequently used Chinese herbs are mainly the liver and kidney meridians. The association rule analysis of Chinese herbs mainly focuses on the combination of Lycium fruit and Eclipta, and Salvia miltiorrhiza and Lycium fruit. The cluster analysis of Chinese herbs results in two groups: group 1includes Rehmannia root,Moutan bark, mulberry fruit,Szechuan lovage rhizome,Angelica sinensis, and white peony root, while group 2 includes processed Polygonum multiflorum,Salvia miltiorrhiza,Lycium fruit,Eclipta,Rehmannia root,Poria, dogwood fruit, water plantain rhizome, and Chinese yam.The complex network analysis identifies a core prescription consisting of Lycium fruit,Eclipta,Salvia miltiorrhiza,Rehmannia root, and processed Rehmannia root.(3)Blood heat and wind dryness syndrome: The top three frequently used Chinese herbs are Rehmannia root(15 times,71.43%),Angelica sinensis(15times,71.43%),and Salvia miltiorrhiza(10 times,47.62%).The top three frequently used Chinese herb functions are nourishing liver and kidney, cooling blood and stopping bleeding, and clearing heat and cooling blood. The four properties of frequently used Chinese herbs are neutral, cold, and warm. The five tastes of frequently used Chinese herbs are sweet, bitter, and pungent. The meridians of frequently used Chinese herbs are mainly the liver and kidney meridians. The association rule analysis of Chinese herbs mainly focuses on the combination of fatty and greasy foods, blood deficiency, blood heat, and blood stasis. It is also associated with dysfunction of the five viscera, mainly due to damp-heat caused by spleen and stomach weakness, insufficient essence and blood caused by liver and kidney deficiency, and blood disorders leading to hair loss.(2)The disease mainly manifests as three types of patterns: spleen and stomach damp-heat,liver and kidney deficiency, and blood heat with wind and dryness.Other patterns have also been identified by medical experts, and treatment should be based on pattern differentiation. The treatment for spleen and stomach damp-heat pattern mainly involves the use of Chinese herbs such as Poria,Coix Seed, and Licorice, with the effects of clearing heat and dampness, detoxifying, and promoting diuresis. The core prescription includes Poria,Coix Seed,Atractylodes,Licorice, and Alisma. The treatment for liver and kidney deficiency pattern mainly uses Chinese herbs such as Ligustrum Seed,Eclipta, and Rehmannia, with the effects of nourishing liver and kidney, cooling blood and stopping bleeding, and clearing heat and cooling blood. The core prescription includes Poria,Coix Seed,Atractylodes,Licorice, and Alisma. For the blood heat with wind and dryness pattern, the most frequently used Chinese herbs are Rehmannia and Angelica, and the core prescription includes Rehmannia,Angelica, and Chuanxiong. For other patterns, the main Chinese herbs used are Rehmannia and White Peony, and the core prescription includes Angelica,Atractylodes,Chuanxiong,Licorice,Astragalus, and Rehmannia. |
