The Studyof Prescription Rules Of Chinese Medicine For Professor Xuan Guowei In Treating Pediatric Alopecia Areata Based On The Data Mining Technology

ObjectiveApplied TCM inheritance support system V2.5(TCMISS V2.5)software to summarize the clinical experiences,traditional Chinese medicine prescription rules for Prof.XUAN Guowei in treating pediatric alopecia areata(AA).And explored the new prescriptions from the rules excavated,discussed their applicability to develope a new TCM diagnosis and treatment idea,thereby to improve the clinical treatment level for AA in children.Finally,layed down the foundation for inheriting the academic thought of Prof.XUAN,and provided materials for the study abuot Lingnan Skin Diseas,it helped pubilc health via inherting the academic experiences of masters of Chinese medicine.MethodsThe subject was divided into two parts: literature review and data mining research.And literature review consist of traditional Chinese medicine and western medicine.The former traced the name,etiology,pathogenesis and treatment of AA from the ancient books.The latter summarized the etiology,pathogenesis,clinical manifestations and treatment of AA.Thereby better understanded the current situation of diagnosis and treatment of AA in children.Based on the collection of TCM medical records with pediatric AA during 2007 and 2017,experienced the diagnosis and treatment by Prof.XUAN,data mining applied TCMISS V2.5with software’s technology--requency stistical methods,complex system entropy class methods,improving mutual infomation entropy method,and unsurpervissed hierarchial clustering analysis and other data mining methods,for the data analysis about prescriptions and Chinesemedications.Combined with the reward of learning with Prof.XUAN to explore the herbs rules and experiences for Prof.XUAN in treating pediatric AA,and discover new prescriptions that contain hidden experiences.ResultsA total of 166 cases of AA with children were collected in the past10 years,including 86 males and 80 females,with a ratio of 1.08:1.The average age was 8.40±3.58 years old,of which the youngest was 3 months and the oldest was 14 years old.65.7% cases of age concentrated at school ages.In accordance with the diagnostic criteria of AA with TCM syndromes,of the 166 cases,spleen and kidney deficiency syndrome took up98 cases,liver and kidney deficiency syndrome occupied 50 cases,bloodheat-promoting-wind syndrome had 12 cases,and 6 cases of Qi-blood deficiency syndrome were included.Followed-up the 137 cases of pediatric AA that Prof.XUAN treated with Chinese medicine.The course of treatment was from 1 month to 3 years,the cure rate was 73.7%,and the effective rate was 88.32%(healed + markedly effective)without any side effect.121 cases(healed and markedly effective)were screened from the 166 cases in according to inclusion criteria,and 278 prescriptions were included in the data analysis.There were 106 kinds of herbs.Ten herbs with the highest frequency of application: licorice,dodder,poria,pine needles,tree peony bark,Ganoderma lucidum,yam,dogwood,dandelion,rehmannia.Among them,poria,tree peony bark,yam,dogwood and rehmannia are the constituents of Liuweidihuang Pills.Pine needles,Ganoderma lucidum,and dandelion are the special herbs used by Prof.Xuan to treat hair loss diseases.Licorice and dodder are beneficial to Qi and kidney function.Got the core prescription with the largest correlation coefficient:Dogwood,dodder,Poria,licorice,pine needles,dandelion,yam,Ganoderma lucidum,peony bark,which were benefit to regulate the function of liver,spleen and kidney.Together with the syndrome differentiation,it revealed that the deficiency of spleen and kidney is the major pathogenesis of pediatric AA.The analysis about Four properties and Five taste showed that mild Chinese medicine accounting for the largest percentage,followed by the cold medicine and warm medicine.The sweet taste of herbs accounted the vast majority.Thechannel-tropism of herbs were kidney,liver and spleen in order.Quantitative analysis found that the amount of Astragalus fluctuated in10-60 g.Most herbs had the basis dosage on 10 g.The main types of traditional Chinese medicine was tonifing herbs,accounting for about52.70%.Others commonly used included heat-clearing herbs,water-dipping osmotic herbs,Pinggan Xifeng herbs,Huoxue herbs,anti-rheumatic herbs,Jiebiao herbs,Xiaoshi herbs,Soothe the nerves herbs,Astringents herbs,Liqi herbs and so on.Some herbs such as bauxite leaves,kelp,and asiaticoside showed Lingnan characteristics.Prof.Xuan treated pediatric AA commonly with the herbs pair like dodder and peony,peony and pine needles,dodder and pine needles.According to the entropy of unsurpervissed hierarchial clustering analysis,a total of six new prescription combinations were gotten: ⑴ new prescription 1: oyster,vitex seed,mulberry leaves,Untreated-Rehmannia,rehmannia and eclipta:⑵ new prescription 2: pine needles,fried hawthorn,thin cover Ganoderma lucidum,pueraria;⑶ new prescription 3: pine needles,pueraria,peony bark,rehmanniae;⑷ new prescription 4: sand Court,kunbu,yam,tribulus terrestris;⑸ new prescription 5: dandelion,yam,tribulus terrestris,alisma,rehmannia,peony bark,eclipta grass: ⑹ new prescription 6:white peony root,dandelion,kunbu,yam,alisma,north ginseng,heterophylla.Conclusion1.Prof.XUAN considered the deficiency of spleen and kidney as the major pathogenesis of pediatric AA.Invigorating spleen and tonifying kidney as the main law of treatment."Balancing the yin and yang,Treating both symptoms and Nature" could be the principle of treatment.Emphasizing the combination of dialectic and identifying symptoms.The usage of herbs and their dosage was flexibal.Most herbs used were sweet taste,balanced properties,accepted by children more easily.Tonifing the body,and at the same time applying the treatment to clear heat,Shufeng,Shenshi,Liqi,Xiaoshi,soothe the nerves,etc.,is conducive to improve the treatment of AA in children.Based on the whole to treat children in AA helped to improve the level of diagnosis and treatment.2.Prof.Xuan treated children in AA with dialectical addition and subtraction based on core prescriptions with significantly effective andsafe.The extracted new prescriptions corresponded to different types of syndromes,and did not fully apply to the study population.They could be applied to AA in adult based on clinical judgments and then expanded the scope of use of themselves.3.TCMISS had analyzed the prescription rules from the aspects of herbs frequency,Four properties and Five taste,channel-tropism,herbs dose analysis,and correlation of herbs,it helped to summarize the experiences for Prof.XUAN in treating pediatric AA.This study layed down the foundation for inheriting the academic thought of Prof.XUAN.
