Objective:The purpose of this study is to observe the clinical effect of needle-knife release and conventional acupuncture on the frozen shoulder during the adhesive period,compare the relevant indexes of the two groups of patients before and after treatment,objectively evaluate the difference in curative effect,and explore the possible mechanism of this needleknife therapy for frozen shoulder,providing a basis for selecting more effective treatment methods for frozen shoulder in clinical practice.Methods:A total of 66 patients who were diagnosed with the frozen shoulder in the adhesive period and were treated in the Department of Pain,the First Affiliated Hospital of Shandong First Medical University from December 2021 to December 2022 were collected.The patients were divided into the observation group and the control group by random and controlled methods,with 33 cases each.The observation group was treated with needleknife release acupoint group.The selected acupoints were Yunmen,Jianliao,Jianzhen,and Ashi points.The corresponding acupotomy release sites were the coracoid process,the intertubercular groove,the greater tubercle of the humerus,the infra glenoid nodule,and the taut bands or tenderness points on the muscle.The treatment was once a week,and one week was a course of treatment.The control group was treated with conventional acupuncture,and the acupoints were selected according to the content of"shoulder leakage wind" in the "13th Five Year Plan" teaching material"acupuncture and moxibustion Therapeutics" of the national higher education of the Chinese medicine industry.The treatment was 6 times a week as a course of treatment-After two courses of treatment,observe the changes in shoulder pain score,activity score,and functional score of the two groups of patients,count the overall efficacy of the two groups of patients and record the adverse reactions that occurred during the treatment in detail.SPSS 25.0 was used to analyze the changes in VAS score,ROM score,ADL score,MMT score,ASES function score,and the overall clinical effect of the two groups of patients before and after treatment,and to explore the mechanism of needle-knife release in the treatment of frozen shoulder in the adhesive period,and draw the conclusion of this study.Result:1.There was no significant difference in the baseline data between the observation group and the control group before treatment(P>0.05),including the patient’s gender,age,affected shoulder,course of the disease,VAS score,ROM score,ADL score,MMT score,and ASES function score,suggesting that the two groups of frozen shoulder patients were comparable.2.After treatment,the VAS score,ADL score,ROM score,and ASES function score of patients in the two groups were significantly improved compared with those before treatment,and the difference in every group was statistically significant(P<0.05).The improvement degree of the observation group was more significant,and the difference between the two groups was statistically significant(P<0.05).It is suggested that both needle-knife release and conventional acupuncture treatment can improve the shoulder pain,mobility,and functional activity of patients with frozen shoulders in the adhesion period,and the effect of the needleknife release group is better.3.After the treatment,the MMT score of the patients was not significantly improved compared with that before the treatment(P>0.05),and there was no statistically significant difference between the groups(P>0.05),which suggested that both needle knife release treatment and conventional acupuncture treatment could not effectively improve the muscle strength of the affected shoulder joint.4.After treatment,6 cases in the observation group were cured,2 cases were invalid,and the total effective rate was 93.5%.In the control group,2 cases were cured,4 cases were invalid,and the total effective rate was 86.7%.The difference was statistically significant(P<0.05).It is suggested that the curative effect of needle-knife release on a frozen shoulder in the adhesion period is better than that of conventional acupuncture.Conclusion:Both needle-knife release and conventional acupuncture can effectively treat patients with the frozen shoulder in the adhesion period,and can significantly improve the pain and functional activities around the shoulder of the patients.However,compared with the conventional acupuncture treatment in the control group,the needle-knife release has a better effect and has certain advantages in improving the pain of the affected shoulder,increasing the activity of the affected shoulder,and improving the level of daily function.The needle-knife release has a significant effect on the frozen shoulder in the adhesion period and is worthy of clinical promotion.
