ObjectiveThe nuclear morphological parameters,nuclear numerical density on area and Ki67 proliferation index of adult diffuse glioma were quantitatively analyzed by image analysis technique to explore the diagnostic value of these parameters in different histological grades of diffuse glioma and their significance in the quantitative classification of IDH-wildtype diffuse glioma and IDH-mutant diffuse glioma.Research Contents1.The nuclear morphological parameters,nuclear numerical density on area and Ki-67 proliferation index of oligodendroglioma,IDH-mutant astrocytoma and IDHwildtype diffuse glioma of different histological grades were quantitatively analyzed,and the application value of these parameters in the quantitative diagnosis and histopathological grading of glioma was explored.2.The nuclear morphological parameters,nuclear numerical density on area and Ki-67 proliferation index of diffuse glioma of IDH-mutant and IDH-wildtype were quantitatively analyzed in different histopathological grades to explore the significance of these parameters in the quantitative classification of diffuse glioma of IDH wild type and IDH mutant.Materials and Methods113 cases of diffuse glioma in Nanfang Hospital from January 2017 to December 2022 were collected.Images of glioma were obtained under a 40-fold objective of an optical microscope with a micro-camera.Image-Pro Plus6.0 software was used for quantitative pathological analysis of the collected images to explore the diagnostic value of nuclear morphological parameters,nuclear numerical density on area and Ki67 proliferation index in diffuse gliomas with different histological grades and the significance of these parameters in the quantitative classification of IDH-wildtype and IDH-mutant diffuse gliomas.Result1.Based on morphological observation and immunohistochemical analysis,in most cases,the higher the histological grade of diffuse glioma,the larger the area,perimeter,long axis and short axis,the smaller the axial ratio,RFF and Form PE,the greater the variation degree of nuclear area,circumference,major axis,minor axis,axial ratio and Form PE,the greater the nuclear numerical density on area,and the higher the Ki-67 proliferation index.The regularity of variation degree of Form AR is not obvious.2.In grade 2 or grade 4 diffuse glioma histologically,compared with IDH-mutant diffuse glioma,IDH-wildtype diffuse glioma had larger nuclear area,perimeter,long axis,and short axis,smaller axial ratio,RFF,Form PE,Form AR,and no significant difference in nuclear numerical density on area and Ki-67 proliferation index.In grade 3 diffuse glioma,compared with IDH-mutant diffuse glioma,the nuclei of IDHwildtype diffuse glioma were larger in the long axis,smaller in the short axis,smaller in the axial ratio,RFF and Form PE,and larger in the nuclear numerical density on area,with no significant differences in the proliferation index of area,perimeter,Form AR and Ki-67 proliferation index.Conclusion1.In diffuse glioma,the area,perimeter,long axis,short axis,axial ratio,Form PE,nuclear numerical density on area,and Ki-67 proliferation index of tumor nuclei have important application value in the quantitative analysis of histological grade within the tumor,but are of relatively limited value in differentiating grade 3 from Grade 4 histology of IDH-mutant astrocytoma.2.In quantitative analysis of differentiation between IDH-mutant diffuse glioma and IDH-wildtype diffuse glioma,the area,perimeter,long axis,short axis,axial ratio and Form PE of tumor nucleus have important application value.
