Objective: To observe the clinical effect of combined umbilical application of Yanghe Decoction in the treatment of Hashimoto’s thyroiditis(hypothyroidism stage)with Yang deficiency syndrome,objectively evaluate the clinical effect of internal and external treatment of traditional Chinese medicine on the basis of the treatment of Youjia Le,analyze the theoretical basis and explain the possible mechanism of action,and provide clinical basis for the internal and external treatment of traditional Chinese medicine in the treatment of this disease.Method: According to the random number table method,the patients meeting the inclusion criteria were randomly divided into three groups.On the basis of lifestyle intervention,the control group was given levothyroxine sodium tablets(unimethylle),the treatment group A was given Yang He Tang plus or less on the basis of the control group,and the treatment group B was additionally given warm Tongsan umbilical application on the basis of the treatment group A.All the three groups were observed continuously for 12weeks(4 weeks a course of treatment,a total of 3 courses).The changes of thyroid power level,antibody titer,thyroid color ultrasound and TCM syndrome score before and after treatment were recorded in each group.SPSS software was used for statistical processing of the changes before and after the data,and safety related indicators were detected during the treatment.Results:(1)Comparison of clinical efficacy: the total effective rate of the control group was 53.33%,that of the treatment group A was 76.67%,and that of the treatment group B was 86.67%.The comparison of the total effective rate of the three groups had significant statistical significance(P < 0.01),and the treatment group B was better than the treatment group A,better than the control group.(2)Total score of TCM syndrome: the total effective rate of the control group was 56.67%,that of the treatment group A was80.00%,and that of the treatment group B was 90.00%.After treatment,the score of the syndrome of each group was decreased compared with that before treatment(P < 0.05),and the downward trend of the treatment group B was more significant than that of the treatment group A and the control group,the difference was statistically significant(P <0.01).(3)The level of thyroid power and antibody titer: After treatment,FT3 and FT4 levels were increased,while TSH,TPO Ab and TGAb were decreased,and there was statistical significance in the improvement of different treatment measures(P < 0.05).(4)Thyroid size: The size of the left lobe,right lobe and isthmus of the thyroid in each group after treatment were reduced to A certain extent compared with that before treatment(P <0.05),and there were statistical differences in the improvement effect of different treatment measures(P < 0.05),but there was no obvious clinical effect on the improvement of thyroid size in the treatment group B compared with the treatment group A(P > 0.05).In terms of reducing TPO Ab and TGAb levels,there was no significant difference between treatment group A and control group,treatment group B and treatment group A(P>0.05).(5)Safety analysis: In the course of 12 weeks of treatment,vital signs of patients in each group were stable,and no obvious abnormalities were found in safety indicators.Conclusion: In the treatment of patients with Yang deficiency type Hashimoto’s thyroiditis(hypothyroidism stage),Yanghe Decoction combined with umbilical application has significant clinical effect,and has obvious advantages in improving patients’ clinical symptoms,improving thyroid function and reducing thyroid antibody level.Western medicine combined with traditional Chinese medicine has clear curative effect on inhibiting goiter compared with single administration of Unimethylate.The therapeutic effect of applying umbilical cord therapy in this respect needs further study.In summary,the internal and external application of traditional Chinese medicine is safe and has significant clinical efficacy without obvious adverse reactions,so it is worthy of clinical application.
