| ObjectiveTo investigate the status quo of preoperative fear of disease progression in the spouses of lung cancer patients and explore the influencing factors of preoperative fear of disease progression in the spouses of lung cancer patients.MethodsThe spouses of lung cancer patients from October 2021 to May 2022 in a provincial tertiary A-level cancer hospital were selected as the research objects by cross-sectional survey.The sample size was calculated according to Kendall’s sample rough estimation method,and the self-designed general information questionnaire,the Fear of Progression Questions for Partners(FOP-Q-SF/P),the Chinese version of the Distress Disclosure Index(DDI),the Chinese version of the Quality of Relationship Index(QRI)The Chinese version of Family Hardiness Index(FHI)and the Chinese version of Perceived Stress Scale(PSS)were investigated.Mean±standard deviation was used to describe measurement data.Frequency and constituent ratio were used to describe the count data.T-test and variance analysis were used to compare the preoperative fear of disease progression among the spouses of lung cancer patients with different demographic characteristics and disease characteristics.Spearman correlation was used to analyze the correlation between preoperative fear of disease progression and DDI,QRI,FHI,PSS of spouses of lung cancer patients.Multiple linear stepwise regression analysis were used to analyze the influencing factors of preoperative fear of disease progression of spouses of lung cancer patients,and P<0.05 was considered statistically significant.Results1.A total of 233 questionnaires were sent out in this survey,and 233 questionnaires were recovered with a recovery rate of 100%,among which there were 5 invalid questionnaires and 228 valid questionnaires,with effective recovery of 97.9%.2.The preoperative Fear of Progression Questions for Partners score of the spouses of lung cancer patients was(35.40±11.55),of which,the family health dimension score was(25.16±8.41),and the social function dimension score was(10.28±5.37).The item 3"fear of lover’s pain"had the highest score(3.73±1.18).The item 1"Feeling anxious at the thought that a loved one’s illness may progress"scored second(3.46±1.25).The Item 10"Fear that drugs will harm your loved one’s health"ranked third(3.43±1.27).3.The results of single factor analysis showed that the spouses of lung cancer patients with different gender,age,education level,number of existing children,type of medical insurance,family monthly income,time from diagnosis to operation,primary,non minimally invasive surgery,first surgery,understanding of the condition,burden of care had different degrees of Fear of Progression Questions for Partners.The difference was statistically significant(P<0.05).4.The results of Spearman correlation analysis showed that the family health dimension and social function dimension of the Fear of Progression Questions for Partners scale were correlated with the Distress Disclosure Index,The Quality of Relationship Index,Family Hardiness Index,and Perceived Stress Scale(P<0.05).The total scores of Distress Disclosure Index,The Quality of Relationship Index and Family Hardiness Index were negatively correlated with the Fear of Progression Questions for Partners(r=-0.877,-0.796,-0.856,P<0.05).Among them,the three dimensions of Family Hardiness Index:responsibility,control and challenge were negatively correlated with the Fear of Progression Questions for Partners(r=-0.698,-0.539,-0.612,P<0.05);The total score of Perceived Stress Scale was positively correlated with the progress of Fear of Progression Questions for Partners(r=0.878,P<0.05).Among them,tension and loss of control,as two dimensions of Perceived Stress,were positively correlated with the Fear of Progression Questions for Partners(r=0.678,0.603,P<0.05).5.The results of multiple stepwise linear regression analysis showed that a total of9 variables including the burden of care,the average monthly income of the family,gender,age,primary lung cancer,pain exposure,intimacy,family tenacity,and perceived pressure entered the regression equation(R=0.920,R~2=0.846,after adjustment,R~2=0.842,F=201.901,P<0.001).These variables accounted for 84.2%of the variation in the degree of disease progression of the spouses of lung cancer patients.Conclusion1.In this study,56.1%of lung cancer patients’spouses had a Fear of Progression Questions score that exceeded the norm(≥34 points)The spouses of lung cancer patients generally have the fear of disease progression before surgery.In clinical practice,we should focus on and effectively reduce the fear of disease progression of the spouses of lung cancer patients in order to improve the quality of life of patients and their families.2.Primary lung cancer,female,low monthly income per capita family,the younger the age,the heavier the burden of care,and the greater the fear of the spouses of lung cancer patients.In clinical practice,targeted measures should be taken actively.It is necessary to pay attention to the risk factors that affect the fear of disease progression of the spouses of lung cancer patients mentioned above,so as to reduce the fear of disease progression of the spouses of patients and improve the quality of life.3.The higher the perceived pressure,the greater the degree of disease progression fear of the spouse of lung cancer patients before surgery,the more obvious the expression of pain,the higher the degree of intimacy satisfaction,the stronger the family tenacity,and the lower the degree of disease progression fear.Clinical intervention on the progress of disease fear of the spouses of lung cancer patients should be implemented early to reduce the level of fear and improve the quality of life. |
