Objective:To investigate the status of fear of cancer recurrence and dyadic coping of lung cancer patients and their spouses,analyze the Influencing factors of fear of cancer recurrence of lung cancer patients and their spouses.Using actor-partner interdependence model to analyze the predictive effect of dyadic coping on fear of cancer recurrence of both spouses.and provide theoretical basis for scientific development of nursing intervention programs.In order to provide theoretical basis for making scientific nursing intervention programs,improve the dyadic coping level of lung cancer patients and spouses,reduce the level of fear of cancer recurrence of patients with lung cancer and spouses,and improve the quality of life of the couple.Method:Data were collected with a convenience sampling method.300 pairs of patients with lung cancer and their spouses hospitalized in two third class hospitals in Henan Province from October 2021 to October2022 were selected as the research objects.Patients and their spouses completed the General Information Questionnaire,Dyadic Coping Inventory,Fear of Progression Questionnaire-Short Form,Fear of Progression Questionnaire-Short Form/Partner.Using SPSS 25.0 and AMOS 24.0 software for statistical analysis data analysis with the methods of descriptive statistics,t test,analysis of variance,pearson correlation,multiple linear regression,structural equation model.Results:1.The score of fear of cancer recurrence of patients with lung cancer and their spouses.The total score of fear of cancer recurrence of lung cancer patients was(40.06?6.50)points,the mean score of physiological health dimension was(21.73?4.48)points,the mean score of social family dimension was(18.33?3.36)points.The total score of fear of cancer recurrence in spouses was(41.01?6.68)points,the mean score of physiological health dimension was(27.34?5.37)points,the mean score of social family dimension was(13.67?3.06)points.There were significant differences in the scores of fear of cancer recurrence between lung cancer patients and spouses(P<0.05).2.The score of dyadic coping in of patients with lung cancer and their spouses.The total score of dyadic coping of lung cancer patients was(122.14?10.67)points,the mean score of stress communication was(25.74?4.47)points,the mean score of support dyadic coping was(32.77?5.12)points,the mean score of delegated dyadic coping was(14.53?3.02)points,the mean score of negative dyadic coping was(31.95?3.85)points,the mean score of common dyadic coping was(17.15?3.90)points.The total score of dyadic coping in spouses was(121.92 ? 11.02)points,the mean score of stress communication was(25.07?4.83)points,the mean score of support dyadic coping was(32.34?5.59)points,the mean score of delegated dyadic coping was(14.99?2.82)points,the mean score of negative dyadic coping was(32.48?4.33)points,the mean score of common dyadic coping was(17.04?3.87)points.Paired T-test showed that there was significant differences in the scores of stress communication,delegated dyadic coping and negative dyadic coping between lung cancer patients and spouses(P<0.05),but there was no significant differences in the scores of support dyadic coping and common dyadic coping between lung cancer patients and spouses(P>0.05).3.Correlation analysis of fear of cancer recurrence and dyadic coping of patients with lung cancer and their spouses.The lung cancer patient’s fear of cancer recurrence was positively correlated with the spouse’s(r=0.150~0.397,P<0.01).Patient’s positive dyadic coping was positively correlated with the spouse’s(r=0.270~0.540,P<0.01),patient’s positive dyadic coping was negatively correlated with the spouse’s negative dyadic coping(r=-0.181~-0.469,P<0.01).Patient’s negative dyadic coping was negatively correlated with the spouse’s positive dyadic coping(r=-0.285~-0.240,P<0.01),patient’s negative dyadic coping was positively correlated with the spouse’s negative dyadic coping(r=0.374,P<0.01).The lung cancer patient’s positive dyadic coping was positively correlated with the spouse’s.The positive dyadic coping of lung cancer patients and their spouses were negatively correlated with the fear of cancer recurrence of lung cancer patients and their spouses(r=-0.402~-0.476,P<0.01).The negative dyadic coping of lung cancer patients and their spouses were positively correlated with the fear of cancer recurrence of lung cancer patients and their spouses(r=-0.321~-0.506,P<0.01).4.Analysis on the influencing factors of fear of cancer recurrence of patients with lung cancer and their spouses.The results of multiple linear regression analysis showed that the influencing factors of fear of cancer recurrence of patients with lung cancer included: age,gender,education level,duration of illness,family income and cancer stage;the influencing factors of spouse’s fear of cancer recurrence included: age,sex,education level,and marriage age.5.The results of actor-partner interdependence model of dyadic coping and fear of cancer recurrence in patients with lung cancer and their spouses.The positive dyadic coping of lung cancer patients and spouses had a negative effect on their own fear of cancer recurrence(P<0.01);the negative dyadic coping of lung cancer patients and spouses had a positive effect on their own fear of cancer recurrence(P<0.01).The positive dyadic coping of lung cancer patients and spouses can negatively predict the level of fear of cancer recurrence of the other party(P<0.01);the negative dyadic coping of lung cancer patients and spouses can positively predict the level of fear of cancer recurrence of the other party(P<0.01).Conclusion:1.Both lung cancer patients and spouses have high level fear of cancer recurrence,and the fear of cancer recurrence of spouses is higher than that of patients.2.The scores of dyadic coping of patients with lung cancer and their spouses are at amedium level,but it still needs to be improved.3 Age,gender,education level,duration of illness,family income and cancer stage are important factors influencing the fear of cancer recurrence in lung cancer patients;age,sex,education level,and marriage age are important factors influencing the fear of cancer recurrence in spouses.4.The level of fear of cancer recurrence of patients with lung cancer and their spouses are affected by their own and their partners’ dyadic coping styles.The higher the level of positive dyadic coping,the lower the level of fear of cancer recurrence of themselves and their spouses.The higher the level of positive dyadic coping,the lower the level of fear of cancer recurrence of themselves and their spouses.The higher the level of negative dyadic coping,higher the level of fear of cancer recurrence of themselves and their spouses.
