Observation On The Therapeutic Effect Of Buyin Yangye Decoction Combined With Auricular Points On Kidney Yin Deficiency Syndrome Of Premature Ovarian Insufficiency

Objective:To study the clinical efficacy and mechanism of Buyin Yangye decoction combined with auricular point pressing in the treatment of premature ovarian insufficiency with kidney-yin deficiency syndrome,so as to provide scientific basis for the comprehensive treatment of premature ovarian insufficiency with traditional Chinese medicine.Methods:From January 2021 to September 2022,66 patients who met the inclusion criteria of premature ovarian insufficiency of kidney-yin deficiency syndrome were selected from the gynecology clinic of Renai Branch of the first affiliated Hospital of Guangxi University of traditional Chinese Medicine.The patients were randomly divided into three groups:control group（n=22,excluding 2 cases）,traditional Chinese medicine treatment group（n=22,shedding 1 case,excluding 1 case）,and comprehensive treatment group（n=22,shedding 2 cases）were treated with Buyin Yangye decoction combined with auricular points.The changes of serum sex hormones（FSH,LH,E₂）,AMH,ovarian volume,number of antral follicles,TCM syndrome score,MRS score scale and menstruation were observed before and after treatment in the three groups,and the observation indexes were compared and analyzed.Results:（1）Comparison of baseline data before treatment:there was no significant difference in age,course of disease,serum sex hormones（FSH,LH,E₂）,AMH,number of antral follicles,ovarian volume,menstruation,total score of TCM syndrome and MRS score among the three groups（P>0.05）.（2）Sex hormone comparison:FSH and LH in the three groups were significantly lower than those before treatment（P<0.05）.The curative effect of the comprehensive treatment group and the traditional Chinese medicine treatment group was better than that of the control group（P<0.05）.The E₂of the three groups was higher than that before treatment（P<0.05）.The curative effect of the comprehensive treatment group and the traditional Chinese medicine treatment group was better than that of the control group（P<0.05）,and the curative effect of the traditional Chinese medicine treatment group was similar to that of the comprehensive treatment group（P>0.05）.（3）AMH comparison:AMH in the three groups increased in varying degrees（P<0.05）,and there was significant difference between the traditional Chinese medicine treatment group and the control group（P<0.05）,but there was no significant difference between the comprehensive treatment group and the traditional Chinese medicine treatment group and the control group（P>0.05）.（4）Comparison of ultrasonic monitoring indexes:the AFC and OV of the three groups were increased（P<0.05）,and the curative effect of the comprehensive treatment group and the traditional Chinese medicine treatment group was better than that of the control group（P<0.05）.There was no significant difference between the comprehensive treatment group and the traditional Chinese medicine treatment group and the control group（P>0.05）.（5）Comparison of menstruation:the menstruation of the three groups was more regular after treatment.After treatment,the menstrual cycle in the traditional Chinese medicine treatment group and the control group was better than that in the comprehensive treatment group（P<0.05）.The menstrual period and menstrual volume in the traditional Chinese medicine treatment group were better than those in the control group（P<0.05）,and the curative effect was similar to that in the comprehensive treatment group（P>0.05）.After three months of follow-up,the cycle,menstruation and menstrual volume of the traditional Chinese medicine treatment group were better than those of the control group（P<0.05）,but there was no significant difference between the traditional Chinese medicine treatment group and the comprehensive treatment group（P>0.05）.（6）Comparison of the curative effect of TCM syndrome:the total score of TCM syndrome in the three groups was significantly lower than that before（P<0.05）,and the curative effect of comprehensive treatment group and traditional Chinese medicine treatment group was better than that of the control group（P<0.05）.There was no significant difference between the comprehensive treatment group and the traditional Chinese medicine treatment group（P>0.05）.All of them could significantly improve insomnia,dreaminess,palpitation,lumbosacral acidity and softness.（7）Comparison of MRS score scale:the MRS scores of the three groups were all improved（P<0.05）,and the curative effect of comprehensive treatment group and traditional Chinese medicine treatment group was better than that of the control group（P<0.05）.The comprehensive treatment group and the traditional Chinese medicine treatment group had the same curative effect（P>0.05）,and could significantly improve sleep problems,emotional disorders and heart discomfort.（8）Comparison of clinical comprehensive efficacy:the effective rate of the control group was 80%,that of the traditional Chinese medicine treatment group was 95%,and that of the comprehensive treatment group was 95%,the difference was statistically significant（P<0.05）.Conclusion:1.Buyin Yangye decoction combined with ear acupoint and Buyin Yangye decoction combined with fenmatong can reduce the levels of FSH and LH,increase E₂and AMH,increase the number of antral follicles,increase ovarian volume,regulate menstrual disorder,prolong menstruation,increase menstrual volume,and improve the symptoms of low estrogen.The clinical comprehensive effect is similar,it can restore ovarian function to a certain extent,the curative effect is definite,and there is no obvious adverse reaction.It has high safety and is suitable for clinical application.2.There is no significant difference in the clinical comprehensive curative effect between Buyin Yangye decoction combined with auricular point treatment and Buyin Yangye decoction combined with fenmatong,which is better than oral fenmatong alone.In clinical application,the treatment method can be chosen according to the patients’acceptance of fenmatong and auricular points.
