| Objective:To observe the clinical effect of Fengshi Zhuang Medicine Decoction combined with Zhuang Medicine Medicated Thread Moxibustion in the treatment of chronic eczema of hands and feet with damp-heat syndrome.Methods:A total of 128 patients with chronic eczema of hands and feet with dampness-heat syndrome meeting the inclusion criteria were divided into 4groups according to random number table method,with 32 patients in each group.They were combined group(Fengshi Zhuang Medicine Decoction bubble bath + Zhuang Medicine Medicated Thread Moxibustion),Zhuang medicine external washing group(Fengshi Zhuang Medicine Decoction bubble bath),medicinal line moxibustion group(Zhuang Medicine Medicated Thread Moxibustion),and control group(compound Flamethasone ointment external application).The treatment course of the 4 groups was 4 weeks.The scores of Eczema area severity index(EASI),Visual analogue scale(VAS)and Dermatology Life Quality Index(DLQI)score of patients in each group were recorded before treatment,2 weeks after treatment and 4 weeks after treatment.they were used to evaluate clinical response rate.Follow-up 1month after the end of treatment was performed to evaluate recurrence rate.Results:(1)A total of 128 cases were included,of which 4 cases fell off during treatment,and 124 cases were finally completed.There were 30 cases in the combined group,32 cases in the Zhuang medicine external washing group,32 cases in the medicinal line moxibustion group and 30 cases in the control group.There were no significant differences in gender,age,course of disease,Eczema Area and Severity Index score,Visual analogue scale score and Dermatology Life Quality Index score before treatment among the four groups(P>0.05).(2)Comparison of Eczema area and severity index score:Compared before and after treatment in each group,EASI scores in the four groups were decreased after intervention,the difference was statistically significant(P<0.05),indicating that the four treatments could improve the eczema lesions in patients.After 4 weeks of treatment,the scores of the combined group(3.37±2.89),the Zhuang medicine external washing group(5.63±2.69),the medicinal line moxibustion group(6.38±2.87)and the control group(4.80±2.68)were scored.After 4 weeks of treatment,pairwise comparison among groups showed statistically significant differences between combination group and Zhuang medicine external washing group,combination group and medicinal line moxibustion group,combination group and control group,medical-line moxibustion and control group(P<0.05),while pairwise comparison among other groups showed no statistically significant differences(P>0.05).(3)Comparison of Visual analogue scale score: Compared before and after treatment in each group,VAS score of the four groups was significantly decreased after treatment,the difference was statistically significant(P<0.05),indicating that the four treatments could improve the pruritus symptoms of patients.After 4 weeks of treatment,the combined group(0.83±0.59),Zhuang medicine external washing group(1.88±0.87),the medicinal line moxibustion group(2.47±0.95)and the control group(1.43±0.82).There were statistically significant differences between combination group and Zhuang medicine external washing group, combination group and medicinal line point moxibustion group,combination group and control group,and medicinal line moxibustion group and control group(P<0.05),but there was no statistically significant difference between other groups(P>0.05).(4)Comparison of Dermatology Life Quality Index score: Before and after treatment in each group,the quality of life score of the four groups was significantly decreased after intervention,the difference was statistically significant(P<0.05),indicating that the four treatment methods can improve the quality of life of patients.After 4 weeks of treatment,the combined group(2.07±1.41),Zhuang medicine external washing group(3.13±1.41),medicinal line moxibustion group(5.84±1.53)and control group(3.40±1.16)were pin-to-pair comparison,and there was no statistical significance between Zhuang medicine external washing group and medicinal line moxibustion group(P>0.05).Pairwise comparisons between the other groups were statistically significant(P<0.05).(5)Comparison of total effective rate:The total effective rate of the combined group was 93.33%,the total effective rate of the Zhuang medicine external washing group was81.25%,the total effective rate of the medicinal line moxibustion group was75.00%,and the total effective rate of the control group was 86.66%.There were statistically significant differences between the combined group and the Zhuang medicine external washing group and the medicinal line moxibustion group(P<0.05).The difference was not statistically significant(P>0.05),suggesting that the curative effect of the combined group was better than that of the Zhuang medicine external washing group and medicinal line moxibustion group.It is suggested that the curative effect of combination group is equivalent to that of control group.(6)Comparison of incidence of adverse reactions: The incidence of adverse reactions in combination group,the Zhuang medicine external washing group,medicinal line moxibustion group and control group were 13.33%,3.12%,6.25%,16.66%,respectively.The incidence of adverse reactions in the four groups was compared,and the P values were all greater than 0.05,which was not statistically significant.The results showed that there was no significant difference in adverse reactions among the four groups.(7)Comparison of recurrence rate: the recurrence rate was 3.57% in combination group,26.92% in Zhuang medicine external washing group,33.33% in medicinal line moxibustion group and 30.76% in control group.There were statistically significant differences between the combined group and Zhuang medicine external washing group,medicinal line moxibustion group and control group(P<0.05),suggesting that the recurrence rate of the combined group was lower than that of Zhuang medicine external washing group,medicinal line moxibustion group and control group.Conclusion:(1)Fengshi Zhuang Medicine Decoction combined with Zhuang Medicine Medicated Thread Moxibustion can effectively reduce the severity of skin lesions and itching in patients with damp-heat syndrome of chronic eczema on hands and feet,and improve the quality of life of patients.(2)The curative effect of Fengshi Zhuang Medicine Decoction combined with Zhuang Medicine Medicated Thread Moxibustion in the treatment of chronic eczema of hands and feet with damp-heat syndrome was significant(93.33%),which was better than that of Fengshi Zhuang Medicine Decoction alone(81.25%)and Zhuang Medicine Medicated Thread Moxibustion alone(75.00%);The efficacy was similar to that of compound flamethasone ointment(86.66%).(3)The recurrence rate of chronic eczema of hands and feet with dampness-heat syndrome treated by Fengshi Zhuang Medicine Decoction combined with Zhuang Medicine Medicated Thread Moxibustion was significantly lower than the other three groups. |
