Clinical Study Of Qingfei Huatan Decoction In Treating Patients With Phlegm-heat Obstructing Lung Syndrome During Acute Exacerbation Of Bronchiectasis

Objective:Through the observation of the clinical efficacy of Qingfei Huatan Decoction in the treatment of patients with phlegm-heat obstructing lung syndrome in the acute exacerbation stage of bronchiectasis and its influence on the serum inflammatory factors MIP-2,suPAR and sTREM-1,the application value of Qingfei Huatan Decoction in the treatment of bronchiectasis was discussed,and the corresponding theoretical basis was provided for the treatment of bronchiectasis by TCM compound.Methods: A total of 60 patients were selected,all of whom were hospitalized in Ruikang Hospital,Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from February 2022 to November 2022.All the patients met the diagnostic criteria for phlegm-heat obstructing lung syndrome during the acute exacerbation of bronchiectasis.Patients with moderate E-faced score(severity grading of modified bronchiectasis)were randomly divided into control and treatment groups(30 patients in both groups)using a random number table.All patients received the same basic Western medicine treatment.The treatment group was added to the control group on the basis of Qingfei phlegm decoction,and both groups were treated for 14 days.The clinical efficacy,serum levels of macrophage inflammatory protein-2(MIP-2),soluble urokinase-type plasminogen activator receptor(su PAR),and soluble myeloid cell trigger receptor-1(s TREM-1)in the two groups were observed and recorded before and after treatment,as well as adverse reactions occurred in the patients during the inclusion process.Results:(1)Qingfei Huatan Decoction had a good clinical effect on patients with phlegm-heat obstructing lung syndrome during the acute exacerbation of bronchiectasis.The total effective rate of the control group was 85.71%,and that of the treatment group was 92.85%.The total effective rate of the treatment group was significantly better than that of the control group,according to statistical analysis(P<0.05),and the difference was statistically significant.(2)After treatment,the total score of TCM syndrome in both groups decreased compared with that before treatment,and the difference between groups was statistically significant(P<0.05).The decrease was more obvious in the treatment group,which proved that the treatment group was better than the control group in improving the clinical symptoms of patients with phlegm-heat obstructing lung syndrome during the acute exacerbation of bronchiectasis.(3)After treatment,the scores of TCM symptoms in the two groups were compared within the group,P<0.05.The treatment of phlegm-heat obstructing lung syndrome in the acute exacerbation of bronchiectasis could relieve the symptoms of cough,yellow sputum,blood in sputum,fever,chest tightness,dry mouth,short yellow urine and constipation in both groups.After treatment,the scores of TCM symptoms in the two groups were compared between the two groups.In terms of the scores of single symptoms of cough,yellow sputum,blood in sputum and short yellow urine,the differences were statistically significant(P<0.05).The scores of single symptoms of fever,chest tightness,dry mouth and constipation were not statistically significant(P>0.05).The treatment group was better than the control group in improving the symptoms of cough,yellow sputum,blood in sputum and short yellow urine.(4)After treatment,the levels of MIP-2,su PAR and s TREM-1 in serum decreased compared with before treatment,and the decrease was more obvious in the treatment group than in the control group,the difference was statistically significant(P<0.05).(5)No obvious adverse reactions occurred in the course of treatment.Conclusion:(1)Qingfei Huatan Decoction combined with Western medicine in the treatment of patients with phlegm-heat obstructing lung syndrome during the acute exacerbation of bronchiectasis can reduce the clinical symptoms of patients,especially in cough,cough yellow sputum,blood in sputum,short yellow urine symptoms than the control group.(2)Qingfei Huatan Decoction combined with Western medicine in the treatment of patients with phlegm-heat obstructing lung syndrome of acute exacerbation of bronchiectasis was significantly better than the control group in terms of reducing the total score of TCM syndrome and improving the clinical efficacy.(3)Qingfei Huatan Decoction combined with Western medicine can significantly reduce the serum MIP-2,su PAR and s TREM-1 levels in patients with phlegm-heat obstructing lung syndrome during acute exacerbation of bronchiectasis.It can be concluded that Qingfei Huatan Decoction can play a certain synergistic role in anti-infection and eliminating inflammation in patients with phlegm-heat obstructing lung syndrome during acute exacerbation of bronchiectasis.It is worthy to be widely promoted in clinic.
