Objective: In this study,by observing the difference of clinical efficacy of Guiluo’s Yigong Yangbao Decoction and Clomiphene in the treatment of small follicle ovulation infertility of kidney Qi deficiency type,and initially exploring the mechanism of action of Guiluo’s Yigong Yangbao Decoction,it provides a new idea and theoretical basis for the treatment of small follicle ovulation infertility with traditional Chinese medicine(TCM).Methods: In this study,60 eligible patients were selected from the gynecological clinic of Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine.According to the random number table method,they were randomly divided into traditional Chinese medicine treatment group and western medicine control group,with 30 cases in each group.The Chinese medicine treatment group was treated with Chinese medicine(Guiluo’s Yigong Yangbao decoction)from the 5th day of menstruation until ovulation(B ultrasound showed that the follicles disappeared).Western medicine control group: patients were treated with clomiphene on the 5th day of the menstrual cycle,oral administration of 50 mg,once a day for 5 days,if the response was not good,the next cycle could be changed to 100 mg,once a day for 5 days,all the patients were treated for 3 consecutive menstrual cycles.The follicular development,endometrial thickness,sex hormone levels(basic follicle stimulating hormone FSH,basic luteinizing hormone LH,basic serum estradiol E2,and sex hormone progesterone P level on the 7th day after ovulation),TCM syndrome scores,TCM syndrome efficacy,and medication safety were observed before and after treatment in the two groups.The success rate of pregnancy in the two groups was followed up for three months after treatment,and the efficacy of Guiluo’s Yigong Yangbao Decoction in the treatment of infertility with small follicle ovulation was evaluated objectively,and the mechanism was explored.Results: 1.Comparison of basal FSH,LH and E2 values: The basal levels of FSH and LH in the included cases were low.The experimental analysis showed that the two groups could effectively increase the basal levels of FSH and LH in a certain range,and the treatment group had a better effect than the control group(P<0.05).For the basal level of E2,there were significant differences between the two groups before and after treatment(P <0.05),and between the two groups after treatment(P<0.05).According to the data analysis,the treatment group could increase the basal level of E2,and the control group could reduce the basal level of E2.2.Comparison of progesterone P level on the 7th day after ovulation:there was no statistical significance in the comparison of progesterone P between the two groups before and after treatment(P>0.05),intra-group comparison before and after treatment: intra-group comparison between the treatment group and the control group(P<0.05),the difference was statistically significant;After treatment,(P<0.05)was statistically significant.Both groups could improve the level of progesterone P on day 7 after ovulation to a certain extent,and the effect was more significant in the treatment group.3.The comparison of the maximum diameter of dominant follicle in ovulation period: the comparison between the two groups before and after4.treatment was statistically significant(P<0.05),and the comparison between the two groups was(P<0.05),the difference was statistically significant.It is concluded that both groups can improve the maximum diameter of dominant follicle during ovulation,and the effect of the treatment group is more significant than that of the control group.5.There were significant differences in the comparison between the two groups before and after treatment(P<0.05),and the difference was statistically significant between the two groups after treatment(P<0.05).Observation and analysis of the data showed that the treatment group could increase the endometrial thickness,while the control group could make the endometrial thinning.6.Comparison of TCM syndrome scores between the two groups: there were significant differences between the two groups(P<0.05).After treatment,the difference between groups was statistically significant(P<0.05).According to the data analysis,the improvement effect of TCM syndrome score in the treatment group was more significant than that in the treatment group.Comparison of the efficacy of TCM syndroms: the effective rate of the treatment group was 80.0%,and that of the control group was 46.67%,P<0.05,indicating that the efficacy of TCM syndroms in the treatment group was more significant than that in the control group.7.During treatment and 3 months after treatment,there were 18 cases of pregnancy in the two groups,including 13 cases in the treatment group and 5cases in the control group.The pregnancy rate in the treatment group was43.33% and 16.67% in the control group,P<0.05,indicating that the pregnancy rate in the treatment group was higher than that in the control group.Conclusion(s): Guiluo’s Yigong Yangbao decoction has significant clinical efficacy in the treatment of small follicle ovulatory infertility with kidney qi deficiency,which can significantly improve the clinical symptoms of infertility patients with kidney qi deficiency,increase the basal levels of FSH,LH and E2,increase the luteal phase progesterone P level,increase the diameter of the dominant follicle before ovulation,improve the endometrial thickness,so as to improve the clinical pregnancy rate.Traditional Chinese medicine in the treatment of infertility has obvious curative effect,no obvious toxic side effects,with good safety,and can be widely used in clinical practice. |
