Background:Without premature rupture of membranes(PPROM)is a common disease of obstetrics,its etiology is relatively complex,many think and infection,uterine abnormal pressure,mechanical stimulation and other related factors,the factors work together,for maternal and newborn health will produce tremendous negative influence,a lot of women in traditional society because of premature rupture of membranes,Resulting in maternal preterm birth,various complications,and fetal death.In today’s era,premature rupture of membranes before term is still the most serious problem in obstetrics and gynecology department.Therefore,scholars at home and abroad also attach great importance to the study of this problem.How to timely and accurately according to gestational age,clinical case treatment is still urgently needed to be solved in obstetrics and gynecology,clear PPROM maternal adverse pregnancy outcomes occur status and related influencing factors,the clinical targeted intervention and concrete implementation has important guiding significance,which can develop individualized method in patients with PPROM,improve maternal and child safety.Objective:The incidence,high-risk factors and pregnancy outcomes of preterm premature rupture of membranes admitted to our hospital were retrospectively analyzed.Clinical classification and management of pregnant women with preterm premature rupture of membranes provides a theoretical basis.Methods:A retrospective study was conducted on 100 preterm women who were hospitalized in the obstetrics department of the Peace Hospital Affiliated to Changzhi Medical College from May 2021 to May 2022.Among them,there were 49 cases in the unruptured membrane group(group B)and the premature rupture of membranes group.(Group A)51cases of delivery patients.Meanwhile,in the premature rupture of membranes group,according to the residual amniotic fluid index(AFI)at the time of premature rupture of membranes,they were divided into: AFI≤50mm,,and AFI>50mm,divided into A1 group:21 cases,A2 group: 30 cases;according to the occurrence of histology chorioamnionitis(HCA)HCA/chorioamnionitis(Chorioamnionitis,CCM)divided into: no chorioamnionitis group(group A3): 35 cases,chorioamnionitis group(group A4): 16 cases.By collecting the demographic and clinical data of patients in each group,the high-risk factors,mode of delivery and maternal and infant outcomes of patients with preterm premature rupture of membranes were analyzed and compared.2.SPSS 26.0 was used for statistical analysis.Independent sample t-test was used for univariate measurement indicators,and chi-square test was used for counting data to analyze the differences between the two groups.Multivariate Logistic regression analysis was used for multivariate analysis.P < 0.05 was considered statistically significant.Results:1.There were no significant differences in maternal age,BMI,pregnancy complications,cervical canal length,gestational age and birth weight among the groups(P > 0.05).2.The incidence of prenatal fever,chorioamnionitis,WBC,SAA,and labor latency more than 24 hours were higher in the premature rupture group,and the differences between the two groups were statistically significant(P < 0.05).The incidence of fetal distress in the premature rupture group was high,and the difference between the two groups was statistically significant(P < 0.05).The results of multivariate Logistic regression analysis showed that prenatal fever and white blood cell count were the risk factors for premature rupture of membranes.3.In the premature rupture of membranes group,The results showed that the prenatal fever rate,leukocytes and serum amyloid protein significantly increased in the group with AFI ≤ 50 mm,and the difference was statistically significant(P < 0.05).4.In the premature rupture of membranes group,the incidence of postpartum fever and SAA value in the chorioamnionitis group were higher than those in the nonchorioamnionitis group,and the differences between the two groups were statistically significant(P < 0.05).The incidence of fetal distress and early-onset sepsis in chorioamnionitis group was high,and the difference between the two groups was statistically significant(P < 0.05).Conclusion:1.Premature premature rupture of membranes is easy to lead to prenatal fever,increased incidence of choriochorioamnionitis,and WBC,SAA in the indicators increased.At the same time,the incidence of neonatal fetal distress was significantly increased.The results of multivariate logistic regression analysis showed that prenatal fever and increased leukocyte count were the risk factors for premature rupture of membranes.2.Serum amyloid protein has a certain diagnostic value for PPROM complicated with choriohiringitis.At the same time,among the neonates with PPROM and choriochoroiditis,the incidence of neonatal diseases is high.
