| ObjectiveTo investigate the correlation between medication compliance and quality of life of HIV-positive MSM who have received antiviral therapy in Shenzhen, and to explore the influence of combination of smart drug box and wechat on medication compliance and quality of life of HIVpositive MSM.MethodThis research is mainly divided into two parts. The first part:cross-sectional study, using non-probability sampling method,a questionnaire survey was conducted in Shenzhen Third People’s Hospital Love Clinic from April 2021 to June 2021,using self-designed questionnaires, including general demographic characteristics,HIV treatment Relevant situation,AIDS Treatment Adherence Self-Efficacy Scale Chinese Revised Version(HIV-ASES-V2),World Health Organization Quality Of Life Questionnaire For HIV Brief Version(WHOQOL-HIV BREF),to investigate and analyze the status quo of medication compliance and quality of life in the treatment of HIV-positive MSM,the factors affecting quality of life and the correlation between the two.The second part: Randomized controlled trial, recruited 40 HIVpositive MSM under treatment according to the inclusion and exclusion criteria, recorded medication compliance data for 1 month using a smart pillbox, and then evenly divided them into an intervention group and a control group according to compliance. There were 20 people in each group, and a 3-month intervention trial was started. The intervention group started We Chat reminders, human intervention after continuous missed doses, and We Chat public account article pushes. The control group only used the smart pillbox to record data.Fill in the HIV-ASES-V2 scale and the WHOQOL-HIV BREF scale at the end of the 1st month and the 4th month, and detect the number of CD4+ T lymphocytes(abbreviated as CD4 cell number)and viral load,at the end of the 4th month, the research subjects’ satisfaction with the use of the pillbox and We Chat was also investigated. Then analyze the data, observe the trend of change, and evaluate the effect of the intervention.Epi Data 3.0 software was used to input data,SPSS 25.0 software was used for analysis, and internal consistency test was used to evaluate the validity;Pearson correlation analysis was used for the correlation between the two scales;count data were expressed by frequency and percentage(%),and chi-square test were used for inter-group comparison;measurement data were expressed as mean ± standard deviation,T-test and ANOVA were used for inter-group comparison;stepwise multiple linear regression was used for multivariate analysis.Result1.Correlation analysis of medication compliance and quality of life in HIV-positive MSMA total of 350 questionnaires were distributed this time, and 337 valid questionnaires were recovered, with an effective rate of 96.3%.The reliability of the two scales is good. The survey results show that 44.5% of MSM aged 30~39 years old,67.7% are unmarried, and 34.1% have received treatment for 3-5 years. The score of medication compliance scale was(112.97±15.48)points,and the score of quality of life was(83.66±13.75)points.There was a significant positive correlation between medication compliance and quality of life.Multiple linear regression showed that the factors affecting quality of life were average monthly income,Residence status,HIV treatment spending in the past 1 year, and medication adherence.2.Intervention effect of smart pillbox combined with We ChatA total of 40 subjects were recruited,20 subjects in the intervention group and 20 subjects in the control group,17 subjects in each of the two groups finally completed the experiment after 4 months. In terms of baseline data, there was no statistical significance in the general situation,HIV treatment-related situation, medication compliance data, and quality of life scores between the two groups of subjects(P>0.05),which were comparable. After 3 months of intervention, there was no significant difference between the two groups in the number of CD4 cells, the medication compliance rate recorded in the medicine box,the rate of taking medication on time, and the quality of life(P>0.05).There was a statistically significant difference in the medication compliance rate of days(P<0.05).There was a statistically significant difference in the number of people with good medication compliance before and after the intervention in the intervention group(P<0.05),and there was a statistically significant difference in the medication compliance rate recorded in the medicine box before and after the intervention in the control group(P<0.05).The survey results of the research subjects’ experience of using the smart pill box and the We Chat platform show that the overall satisfaction with the smart pill box is 35.3%,the satisfaction with the stability of the use of the pill box is 32.4%,and the satisfaction with the shape of the pill box is 35.3%.64.7%,the satisfaction with the operation of the medicine box is 50%,and the satisfaction with the function of the We Chat applet is 64.7%.Conclusion1.The quality of life of HIV-positive MSM in treatment is higher, and the average monthly income, living conditions and HIV treatment expenses in the past 1 year are the factors affecting the quality of life.2.There is a significant correlation between medication compliance and quality of life in HIV-positive MSM.The worse medication compliance is,the worse quality of life is.3.The combination of smart pillbox and We Chat can improve the medication compliance of HIV-positive MSM under treatment, and this intervention method is highly feasible and acceptable in this study, but the quality of life has not been significantly improved.Further adjustment and optimization of the intervention program is required. |
