The Screening Value Of Phase Angle And The Osteoporosis Self-assessment Tool For Asians For Osteoporosis

Objective:To explore the diagnostic value of OSTA index and PA in elderly patients with osteoporosis Materials and methods:The clinical data of the patients aged ≥ 60 who were treated in the first affiliated Hospital of Nanchang University from July 2022 to February 2023 were collected.According to the domestic quantitative CT(QCT)osteoporosis(OP)diagnostic criteria,the patients were divided into OP group(n = 38)and non-OP group(n = 42).The body mass index(BMI)and Asian osteoporosis self-screening tool(OSTA)index of the two groups were calculated.The laboratory indexes of serum alkaline phosphatase,serum phosphorus,serum calcium,fasting blood glucose,low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)and total cholesterol were collected.Inbody770 body composition analyzer was used to measure phase angle(PA)and other body composition indexes in both groups.The differences of PA and OSTA index between the two groups were compared,the factors affecting OP were analyzed by binary Logistic regression,and the predictive value of OSTA index and PA alone and in combination was analyzed by ROC curve.Results:(1)The PA and OSTA index were lower in OP group than those in non-OP group,the height,weight,hemoglobin,muscle weight,skeletal muscle weight,body cell weight,fat-free weight,inorganic salt and protein were lower in OP group than those in non-OP group,and the age in OP group was higher than that in non-OP group.(2)The PA and OSTA index were higher in male group than those in female group(p < 0.05).(3)The area under the ROC curve of OSTA index in screening OP is0.803[95%CI(0.706-0.901),p<0.0001],the sensitivity is 68.4%,and the specificity is 83.3%.The best cut-off value is-2.86.The area under the ROC curve of PA in screening OP is 0.788 [95%CI(0.682-0.871),p < 0.0001],the sensitivity is 86.8%,the specificity is 38.1%,and the best cut-off value is 4.65 °.The area under the curve of PA combined with OSTA index for screening OP was 0.823 [95%CI(0.722-0.900),p < 0.0001].There was no significant difference in screening OP between OSTA and PA+OSTA(p = 0.377),and there was no significant difference between PA and PA+OSTA in screening OP(p = 0.261).(4)The area under the curve of OSTA index in men and women was 0.881 and0.751,and the Z statistic was 1.337(p > 0.05).There was no significant difference in the area under the curve of ROC between men and women when using OSTA to screen OP.(5)Binary Logistic analysis showed that OSTA(OR=0.690,95%CI 0.508-0.939)was the protective factor of OP.Conclusion:1.The diagnostic efficiency was not significantly improved when OSTA index was used in combination with PA,but the sensitivity and specificity of screening OP were improved when used together;2.The men and women have the same screening value when OSTA index is used to screen OP;3.OSTA index is a protective factor for OP.
