BACKGROUND & OBJECTIVE:In all regions of the world, the ASR for incidence and mortality is higher in men than in women(male:female ASR ratio between 1.2 and 3.6).It is predicted that between 2020 and 2040,the number of new cases of liver cancer will increase by55.0% per year, and 1.4 million people may be diagnosed with liver cancer in 2040.It is predicted that 1.3 million people may die from liver cancer in 2040(56.4% more than in 2020).As a country with a large population and a high rate of hepatitis B virus infection, liver cancer is currently the fourth most prevalent disease in China, yet liver cancer has the second highest mortality rate in the country. The reason why the mortality rate of liver cancer is so high is because of the tumor’s own malignant rate, high invasiveness, metastasis rate and recurrence rate,in addition to the difficulty of treatment. This project investigates the clinical efficacy of Sophora granules on the prevention of recurrence in patients with microvascular infiltration after hepatectomy for primary hepatocellular carcinoma, and provides further reference for the treatment and research of hepatocellular carcinoma during the TACE treatment period, and explores Sophora granules in the microvascular environment, regulates tumor factor signaling pathways, finds the corresponding receptor targets, inhibits tumor microvascular infiltration status, reverses and eliminates the infiltration status, and provides preoperative treatment and improves microvascular infiltration. The study provides a basic theoretical basis for preoperative treatment and improvement of microvascular infiltration and preoperative descending treatment.Research Objects and Methods:The clinical data of 132 patients with liver malignant tumors admitted to the Hepatobiliary and Pancreatic Surgery Center of the First Hospital of Jilin University from January 2018 to October 2022 were screened, including 108 males and 24 females.Age 31~74 years old, average age(57.12±9.76)years,retrospective analysis. The preoperative AFPindex unit,HBV-DNAvirus quantification, and the postoperative pathology report showed the grade of microvascular infiltration. The first group comparison:According to whether the patients underwent transarterial chemoembolization(T ACE)after surgery, the total sample size patients were divided into TACE group and control group(without TACE),2022 August was the basis point, and the month was the month, and the postoperative patients were followed up with PFS(disease-free progression)and OS(overall survival). The second group comparison: according to whether the patient underwent TACE after surgery, oral Sophora granule was divided into two groups: B1 and B2.The third group comparison: the samples were rearranged into MVI(M1)and MVI(M2),and the samples were screened from the same microvascular invasion grade: Group X without any adjuvant therapy was performed. With group Y(only TACE is performed postoperatively),group Z(TACE and Sophora granules are performed postoperatively).Compared with PFS and OS during the follow-up period,COX survival analysis was performed, and K-M curves were plotted to determine whether there was a significant difference in postoperative PFS and OS between the two groups.Outcome:of the included study132 Patients with microvascular invasion after resection of primary hepatocellular carcinoma are treated with or without postoperative ACE,and untreated group A71 example,B Group 61 example,at follow-up Time system PFS,OS,For the disease-free advanced group A and group B during the follow-up period,x(?)2=7.899,p=0.005< 0.05,Overall survival was x (?)2 = 0.027 and P =0.870 (?) 0.05 in group A and group B Not proof There was a statistically significant difference between the two groups,i.e.,recurrence rates between the two groups is different。 The total sample size depends on the patient Basis for existence or not Oral administration of Sophora granules divide into Group B1(with TACE,There are locust huaier)36case,group B2(with TACE,No locust huaier)35case,the same is based on the counting data for Log-Rank test,For the disease-free advanced B1 group during the follow-up period, the comparison of the B2 group: x(?)2=5.316,p=0.021 (?) 0.05,the significance of the two groups was obvious, and there was a statistical difference, that is,there was a statistically significant difference in the recurrence rate of the two groups. For the overall duration of follow-up Lifetime B1,B2 two Group comparison: x(?)2=0.002,P=0.967 (?) 0.05,there was no statistical difference between the two groups. That is,the survival rate of the two groups of samples is statistically significant Not found There was no clear difference.Rearrange the samples into divisions MVI(M1)and MVI(M2),sample screening from the same microvascular invasion grade: Group X without any adjuvant therapy will be performed45 case, with group Y(TACE only performed postoperatively)29case,group Z(TACE is performed postoperatively And locust earsgranules)25case The three groups were compared in pairs, and the counting data of the three groups were used as Log-Rank test.significance analysis,Group Z and Group X,Chi-square value It was11.378,with significance P=0.001,and there was a statistical difference, and there was a significant difference between the two in the disease-free advanced stage。Group Z was compared with Group Y,and the significance P value was < 0.05,demonstrating a statistical difference between the two。Conclusion:A retrospective study was conducted by selecting the clinical data of 132 patients with postoperative pathological reports confirming microvascular infiltration in primary hepatocellular carcinoma admitted to the Hepatobiliary and Pancreatic Surgery Center of the First Hospital of Jilin University since January 2018 to October2022,and the following conclusions were obtained by survival regression analysis.Sophora granules combined with transcatheter hepatic artery chemoembolization can effectively reduce recurrence and prolong disease-free progression in patients with primary hepatocellular carcinoma with microvascular infiltration after surgery...
