Objective Based on the construction path of the "two guarantees,three orders,and six connections" of Anhui Province’s tightly knit county-level medical community,the operating mechanism and key elements of the leading hospital are summarized and summarized.On this basis,quantitatively analyze the operational efficiency of the leading hospital,clarify the mechanism of influencing factors on operational efficiency,and provide empirical basis for optimizing the operational efficiency of the leading hospital,improving the construction of a close county medical community,and enhancing the overall medical service capacity of the county.MethodsThe DEA-BCC model was used to calculate its operational efficiency in 2021,and the results were evaluated based on three dimensions: region,hospital type,and the maturity of the construction of a compact county medical community.Taking the operational efficiency of the leading hospital as the explanatory variable,select indicators based on both internal and external perspectives of the leading hospital,and construct a Tobit regression model based on the comprehensive efficiency,pure technical efficiency,and scale efficiency of the leading hospital to analyze the factors affecting the operational efficiency of the hospital from multiple levels,both overall and classified,to clarify the direction of the influencing factors.Results1.The average comprehensive efficiency,average pure technical efficiency,and average scale efficiency of the 95 leading hospitals in Anhui Province are 0.673,0.891,and0.753,respectively.A total of 19 leading hospitals have a pure technical efficiency value of 1,accounting for 20.0%.Six leading hospitals are DEA effective,and most leading hospitals are in a state of diminishing returns to scale.2.The average comprehensive efficiency of 31 leading hospitals in northern Anhui,41 in central Anhui and 23 in southern Anhui is 0.653,0.691 and 0.668 respectively;The effective proportion of DEA was 6.4%,7.3% and 4.3% respectively.The operation efficiency of the leading hospital in central Anhui was generally good.3.In different types of hospitals,the DEA efficiency of general hospitals and traditional Chinese medicine hospitals is 3.6% and 10%,respectively.The average operating efficiency of traditional Chinese medicine hospitals is higher than that of general hospitals,and the proportion of diminishing returns to scale in general hospitals is higher than that of traditional Chinese medicine hospitals.4.The average comprehensive efficiency and average pure technical efficiency of the leading hospital that started to build a compact county medical community in 2019 were0.677 and 0.905,respectively,which were higher than those of the leading hospital that started to build a compact county medical community in 2020,but the average scale efficiency was slightly lower.5.The number of active employees and career income have a significant impact on the overall comprehensive efficiency of the leading hospital,and there are differences in influencing factors among different regions and types of hospitals.Among them,the impact of bed utilization rate on comprehensive hospitals and traditional Chinese medicine hospitals is opposite.Conclusion1.The irrational scale of the hospital has led to the overall operating efficiency of the hospital led by the close county medical community in Anhui Province.2.There are differences between different regions and hospital grade types.The overall situation of the central Anhui region and the second-level TCM hospitals is better.3.The construction of a close county medical community has enabled the leading hospital to return to the functional orientation of difficult,critical and critical treatment and promote the rational use of medical resources in the hospital.4.Reduce the scale and technical level to improve the operation efficiency of the leading hospital.5.The factors that affect the overall operational efficiency of the leading hospital and the operational efficiency of different regions and types of hospitals differ.Based on the above conclusions,suggestions are proposed to improve the operational efficiency of the leading hospital from four aspects: improving the talent management mechanism,rationally allocating medical resources,formulating targeted development strategies,and deeply promoting the construction of medical communities.In addition,the construction of a compact county medical community is optimized and improved to achieve a high degree of integration and rational allocation of medical resources within the county.
