Analysis Of Current Situation And Problems Of County Medical Community Construction From The Perspective Of Symbiosis Theory

Objective: To understand the current manifestation of G county medical community,find the problems and deficiencies in the construction process of the medical community,and explore reasonable ways to optimize the development of the medical community,so as to promote the steady development of the county medical community construction.Methods: In this study,field investigation was conducted on G County regional medical community.On the one hand,questionnaire survey was conducted to understand the medical staff’s understanding of the policy and construction status of the medical community.On the other hand,operation data of township health centers in G county medical community from 2018 to 2021 were collected.The statistical analysis method and DEA data envelopment analysis method were used to analyze the questionnaire data and collected operational data respectively,to reflect the current situation and existing problems in the construction of the medical community in G County from both subjective and objective aspects,and to conduct in-depth research combined with the symbiosis theory.Results: According to the results of relevant investigation and operational data analysis,it is found that the main problems in the construction of G county medical community are as follows.(1)Symbiotic unit aspect.The medical staff had a good understanding of the policies of the county medical community and their satisfaction was average.Both leading hospitals and township health centers have the problem of insufficient capacity in the construction of the medical community,and the interest demands of different subjects are different.(2)symbiotic environment.County medical community related policies linkage degree is poor,lack of legislative documents,low policy efficiency.The internal rules and regulations of the medical community are not perfect,the lack of talent echelon construction and effective benefit distribution mechanism,the operation and management process and system are not perfect,the authority is not clear,and the interpersonal relationship is complicated.Public participation is low.(3)symbiotic interface.The information construction of the medical community is backward.Although the information system of all participating units has improved after the implementation of the construction of the medical community,the information interconnection work has made modest progress.The frequency of telemedicine in the medical community is less and the utilization rate of telemedicine system is low.(4)symbiotic mode.After the implementation of the county medical community construction,the cooperation forms and cooperation intensity between the leading hospitals and the participating units are obviously enhanced,but the conflicts of interest are obvious.Unified human resources,unified finance,unified information management and other aspects are not fully realized,and the construction of community of interests and management community needs to be strengthened.(5)Operation efficiency.G The operating efficiency of township health centers in the community of county hospitals is low.Horizontal analysis shows that the effective number of DEA in 15 township health centers of G County in 2021 is 4,accounting for only 26.7%.The average comprehensive technical efficiency of township hospitals with better closeness is higher.The vertical analysis shows that after the implementation of the county medical community construction,although the human,material and financial input of township health centers increased significantly,the number of patients and business income did not increase significantly or even decreased,and the total factor productivity showed a trend of first decline and then rise.Conclusions: The construction of the medical community in G County has achieved initial results,but there are still many problems.According to the current development status of G county regional medical community,the following suggestions are put forward.To improve the policy understanding and ability of the county medical community of symbiotic units;To create an internal and external symbiotic environment suitable for the development of county medical community;To construct a symbiotic interface conducive to the "three-flow" exchange in the medical community;To promote the transformation of county medical communities into mutually-integrated symbiosis models.
