Objective:Intervening in Wilson’s disease-related Osteoporosis with Jiedu Bushen Huoxue Method.Understanding the clinical efficacy of the method in WD-related osteoporosis and its effect on bone metabolism by clinically observing and monitoring the differences in biochemical indexes of bone metabolism(BSP,POSTN,Cathe K),bone mineral density and the related scoring scales[Pain Visual Analogue Scoring Scale(VAS),Oswestry Dysfunction Index(ODI)]before and after treatment,so that to provide a clinical and experimental basis for the treatment of this group of patients in the clinical setting.Methods:1.Grouping:Based on the inclusion and exclusion criteria,the included 60 patients with WD-related osteoporosis were randomly divided into a control group and an observation group,with 30 cases in each group.The two groups were treated with conventional copper expulsion,and the control group was given "calcium carbonate D3+osteotriol+alendronate sodium" orally;the observation group was given"calcium carbonate D3+osteotriol+alendronate sodium+JieDu Bushen Huoxue Method(DaHuang 6g,Huang Lian 6g,Jiang Huang 10g,Jin Qiancao 10g,Ze Xie 10g,ShuDi 15g,DuZhong 15g,Bu Guzhi 10g,Yin Yanghuo 10g,Sang Jishen 15g,Dan Shen 10g,DangGui 15g,NiuXi 10g)".Combined with shedding and rejection criteria,a total of 55 patients were finally collected as the final study subjects,and 28 and 27 cases were respectively collected in the two groups.2.Indicators and efficacy:The treatment lasted for 8 weeks in total.Among them,calcium carbonate D3 was given orally with 1 tablet qd,osteopontin 1 capsule bid orally,alendronate 1 tablet qw orally,and detoxification,kidney and blood activation formula was given in one dose in the morning and one dose in the evening,divided into two doses.Before and after treatment,all subjects were monitored for efficacy indicators and completed relevant scoring scales:(1)Bone density(using dual-energy X-ray absorptiometry,selected sites:lumbar L1-L4,double hip joints)(2)Biochemical markers of bone metabolism:Bone sialoprotein(BSP),periosteal protein(POSTN),Cathepsin K(Cathe K)(3)Visual analog scale of pain(VAS),Oswestry dysfunction index(ODI)(4)WD Rating Scale Scores(UWDRS)Results:1.Comparison of basic data:statistical tests of gender,age,duration of disease,weight,height,BMI,and calcium intake of the patients included in the study showed no significant differences in the basic data of the two groups(P>0.05).2.Bone density comparison:there was no significant difference in the lumbar L1-4 and bilateral femoral bone density values between the two included groups before treatment(P>0.05);after treatment,the lumbar L1-4 and bilateral femoral bone density values increased in both groups(P<0.01),and the increase was more pronounced in the observation group than in the control group,and the difference was statistically significant(P<0.01).3.Comparison of biochemical indexes of bone metabolism:the differences in BSP,POSTN,and Cathe K between the two groups of patients included before treatment were not statistically significant(P>0.05);the values of BSP,POSTN,and Cathe K decreased in both groups after treatment(P<0.01),and the decrease was more pronounced in the observation group than in the control group,and the difference was statistically significant(P<0.01).4.Comparison of pain visual analogue scale(VAS)and Oswestry dysfunction index(ODI):there was no significant difference in the scores of the relevant scales between the two included groups before treatment(P>0.05),and the scores of VAS and ODI decreased in both groups after treatment compared with those before treatment(P<0.01),and the decrease was more obvious in the observation group than in the control group,and the difference was statistically significant(P<0.01).5.Comparison of WD rating scale scores(UWDRS):there was no significant difference in UWDRS scores between the two groups before treatment(P>0.05),and UWDRS scores decreased in both groups after treatment compared with those before treatment,and the observation group was significantly lower than the control group(P<0.01).6.Adverse reactions:There was no difference in the incidence of adverse reactions between the 2 groups of patients(P>0.05).Conclusion:The Jiedu Bushen Huoxue method can significantly improve the clinical symptoms of WD patients with fewer side effects.The method may be used to reduce serum BSP,POSTN,and Cathe K levels by regulating signaling pathways such as PI3K-Akt-m TOR to improve bone metabolism disorders in patients with WD-related osteoporosis.
