| Objective:To explore the difference in the short-term clinical efficacy of young and middle-aged neck of femur fractures(FNF)treated with neck of femur dynamic cross nailing system(FNS)and cannulated compression screw(CCS),so as to provide reference for clinicians..Methods:From September 2019 to September 2022 in Baoding First Central Hospital,68 patients with neck of femur fracture who underwent FNS or CCS internal fixation were retrospectively analyzed.According to two different surgical methods, the patients were divided into FNS group and CCS group. There were 34 cases in FNS group and CCS group respectively.Retrospective analysis of patients’ general clinical data, preoperative and intraoperative surgical data, and early postoperative evaluation indicators, using SPSS 25.0 to appropriately evaluate the collected data, and then draw conclusions. Results:There was no statistically significant difference in general data(age, gender, injury factors, injury side, comorbidities,Garden classification, etc.)between the FNS group and the CCS group, which was consistent with the baseline data of the comparative study; There was no significant difference in the intraoperative evaluation index(Garden index)between the two surgical methods(P>0.05).The difference in surgical incision length, intraoperative bleeding, and average surgical time was clinically significant(P<0.05).The CCS group had shorter surgical incisions and less intraoperative bleeding compared to the FNS group, but the FNS group had shorter average surgical time; The difference in postoperative evaluation indicators(partial weight bearing and complete weight bearing time)between the two surgical methods was statistically significant(P<0.05).The FNS group had shorter postoperative partial weight bearing and complete weight bearing time compared to the CCS group; There was no significant difference in VAS scores within 24 hours and 72 hours after surgery between the two groups of patients(P>0.05),and there was no statistically significant difference in average hospital stay and early postoperative complications between the two groups of patients(P>0.05);The patients in the two groups were followed up for 6 months. At the 6th month of follow-up,the changes of neck shaft angle, the degree of shortening of neck of femur and Harris score of hip joint in the two groups were statistically significant(P<0.05).FNS group was better than CCS group. Conclusions:FNS is a new, simple and effective internal fixation method for the treatment of young and middle-aged patients with FNF.Compared with CCS,FNS has shorter operation time, earlier partial and complete loading time after surgery, stronger stability, smaller changes in cervical trunk Angle and shorter shortening degree of femoral neck.Harris score at 1,3 and 6 months after surgery showed that hip function was better in FNS group at the same time. But the CCS group had shorter incisions and less intraoperative blood loss. |
