According to the results of the seventh population census,the number of elderly people over 60 years of age in China has exceeded 260 million,accounting for 18.7% of the total population.The size of the elderly population has increased by 5.44% in the decade from 2010 to 2020,and the trend of social aging is continuing to increase.With the growth of age and the weakening of physical function,a large number of elderly people have reduced their ability to live due to aging,disease or disability.Their demand for comprehensive services such as medical care,elderly care and nursing is urgent.At present,how to provide effective medical and elderly care services for the elderly has become one of the key directions of policy and research.The integration of medical care and elderly care is an elderly care concept that integrates medical and elderly care resources to provide comprehensive services for the elderly population.It is an important policy for China to address the aging population.The disabled elderly are the key service targets of the combination of medical care and elderly care.Due to their limited daily life activities,cognitive abilities,or cognitive abilities,they have a higher demand for medical and elderly care than ordinary elderly people.According to China’s "9073" elderly care plan,medical and elderly care integrated elderly care services mainly rely on medical institutions,elderly care institutions,and communities to carry out.However,currently,the allocation of medical and elderly care resources is insufficient,medical and elderly care service projects are formalized,and service levels are uneven.These practical conditions indicate that the supply of medical and elderly care integrated elderly care services for disabled elderly people is insufficient,supply and demand are mismatched,and the elderly care service system is not yet sound,which needs to be further improved.This article starts from two levels: macro resource allocation and micro service description,and studies the current situation of service demand,policy supply,service mode,and service content.It fully uses questionnaire survey and interview methods to conduct research on disabled elderly people and medical care institutions in Chengdu.It describes and analyzes the differences in service demand with different degrees of disability and the emphasis of different types of institutions on medical care services for disabled elderly people,Explore the practical difficulties and causes of integrating medical care and elderly care services for disabled elderly people.Research has found that disabled elderly people have a high dependence on their demand for medical and long-term care services.However,the uneven allocation of resources for medical and elderly care integrated services for disabled elderly people,the mismatch between supply and demand of medical and elderly care services,and the inefficient integration of medical and elderly care resources have led to incomplete and low quality and efficiency of medical and elderly care integrated services for disabled elderly people,making it difficult to meet the multi-level and diversified elderly care needs of disabled groups.Analyzing the reasons from the perspective of government functions and welfare pluralism,the problem points to unclear policy guidance,lack of unified disability definition standards and service standards;Limited financial investment and imperfect market mechanism;The government has multiple management positions,and the division of labor between medical and nursing care is not clear;The talent cultivation system is not sound and lacks effective performance incentive mechanisms.This article proposes three countermeasures: improving the integrated medical and elderly care service system for disabled elderly people,promoting effective integration of medical and elderly care services for disabled elderly people,and improving the construction of professional talent teams.It is recommended that government departments play a leading role,formulate unified industry standards,actively play the role of financial guidance,and build a multi entity collaborative mechanism;Promote the improvement of quality and efficiency of medical and elderly care services through graded care for disabled elderly and "smart elderly care";Improve the talent training system,professional title level,and performance evaluation system for the integration of medical and nursing care,and improve the construction of professional talent teams.
